บทคัดย่อ
ผู้ป่วยโรคหัวใจและหลอดเลือดใช้ยาที่ซับซ้อน เสี่ยงต่อการเกิดปัญหาจากการใช้ยา (DRPs) ซึ่งเพิ่มภาระค่าใช้จ่ายในการรักษา เนื่องจากข้อมูลผลลัพธ์ทางเศรษฐศาสตร์จากการบริบาลทางเภสัชกรรมในกลุ่มนี้ยังมีจำกัด การประเมินต้นทุนที่หลีกเลี่ยงได้ (cost avoidance) จึงสำคัญเพื่อสะท้อนมูลค่าทางวิชาชีพและใช้สนับสนุนการจัดสรรอัตรากำลังเภสัชกร การศึกษานี้จึงมุ่งวิเคราะห์ประเภทของ DRPs จากการแทรกแซงของเภสัชกร (pharmacist intervention) และประเมินมูลค่าต้นทุนที่หลีกเลี่ยงได้ในผู้ป่วยนอก ณ ศูนย์หัวใจสิริกิติ์ฯ มหาวิทยาลัยขอนแก่น วิธีดำเนินการวิจัย: เป็นการศึกษาแบบเก็บข้อมูลย้อนหลัง (retrospective study) ระยะเวลา 1 ปี (1 ตุลาคม 2563 ถึง 30 กันยายน 2564) โดยรวบรวมข้อมูลจากใบสั่งยาผู้ป่วยนอกที่พบ DRPs จากแบบบันทึก pharmacist intervention และความคลาดเคลื่อนจากการบันทึกข้อมูลในระบบคอมพิวเตอร์ (Key-in error) วิเคราะห์ประเภทของปัญหาและยาที่เกี่ยวข้องด้วยสถิติเชิงพรรณนา พร้อมทั้งคำนวณต้นทุนที่หลีกเลี่ยงได้โดยใช้สมการอิงค่าความน่าจะเป็นของการเกิดเหตุการณ์ไม่พึงประสงค์ ผลการวิจัย: จากการตรวจสอบใบสั่งยาทั้งหมด 80,737 ใบ พบความคลาดเคลื่อนแบบ Key-in error 9,400 ครั้ง (ร้อยละ 11.64) และพบ DRPs ที่ต้องให้ pharmacist intervention 860 ครั้ง (ร้อยละ 1.06) ประเภท DRPs ที่พบบ่อยที่สุดคือ การให้คำแนะนำและติดตามผล (ร้อยละ 24.77) และปัญหาด้านขนาดยาที่ไม่เหมาะสมทั้งสูงและต่ำเกินไปรวมกัน (ร้อยละ 24.18) ยาที่พบปัญหามากที่สุด 5 อันดับแรก ได้แก่ warfarin atorvastatin carvedilol insulin และ amiodarone ตามลำดับ ด้านผลลัพธ์ท างเศรษฐศาสตร์ พบว่า pharmacist intervention สามารถประหยัดต้นทุนที่หลีกเลี่ยงได้รวมทั้งสิ้น 10,337,200 บาทต่อปี แบ่งเป็นต้นทุนจากการแก้ไข Key-in error 7,205,200 บาท และจากการแก้ไข DRPs 3,132,000 บาท โดยปัญหาขนาดยาสูงเกินเป็นประเด็นที่สร้างมูลค่าต้นทุนที่หลีกเลี่ยงได้สูงที่สุด สรุปผลการวิจัย : การบริบาลทางเภสัชกรรมที่ผ่านการตรวจสอบใบสั่งยาและการทำ pharmacist intervention อย่างเป็นระบบ ช่วยป้องกันและแก้ไข DRPs และป้องกันต้นทุนที่หลีกเลี่ยงได้ซึ่งช่วยยกระดับความปลอดภัยและคุณภาพการรักษาในผู้ป่วยในโรคหัวใจและหลอดเลือด
