ผลลัพธ์การให้บริการเภสัชกรรมทางไกลร่วมกับการส่งยาทางไปรษณีย์ในผู้ป่วยมะเร็งเต้านม

บทคัดย่อ
ความรู้และความต่อเนื่องในการใช้ยาต้านฮอร์โมนเป็นปัจจัยสำคัญที่กำหนดผลลัพธ์การรักษาในผู้ป่วยมะเร็งเต้านม การบูรณาการบริการเภสัชกรรมทางไกลร่วมกับการส่งยาทางไปรษณีย์ได้รับการเสนอเป็นแนวทางหนึ่งในการเพิ่มการเข้าถึงบริการทางเภสัชกรรมและลดข้อจำกัดด้านการเดินทาง วัตถุประสงค์: เพื่อประเมินผลของการบูรณาการบริการเภสัชกรรมทางไกลร่วมกับการส่งยาทางไปรษณีย์ต่อความรู้เกี่ยวกับยา ปัญหาทางยา (Drug-Related Problems; DRPs) ความร่วมมือในการใช้ยา ความพึงพอใจของผู้ป่วย และมูลค่าการประหยัดค่าใช้จ่ายในการเดินทางในผู้ป่วยมะเร็งเต้านมที่ได้รับยาต้านฮอร์โมน วิธีการศึกษา: การศึกษากึ่งทดลองแบบไปข้างหน้า โดยดำเนินการที่โรงพยาบาลขอนแก่น ระหว่างวันที่ 31 มีนาคม–31 ธันวาคม พ.ศ. 2568 ผู้ป่วยจำนวน 85 รายที่ได้รับยาต้านฮอร์โมนเข้าร่วมโปรแกรมเภสัชกรรมทางไกลที่บูรณาการกับการส่งยาทางไปรษณีย์ภายใต้กรอบรูปแบบการจ่ายยาแบบบูรณาการภายในสถานพยาบาล (Medically Integrated Dispensing; MID) ประเมินความรู้ด้วยแบบทดสอบ 5 ข้อ ความร่วมมือในการใช้ยาวัดโดย Modified Medication Possession Ratio (mMPR) ความพึงพอใจประเมินด้วยมาตราส่วน Likert 5 ระดับ และบันทึกค่าใช้จ่ายในการเดินทาง วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและ Wilcoxon signed-rank test โดยกำหนดระดับนัยสำคัญที่ p < 0.05 (two-tailed) ผลการศึกษา: ผู้เข้าร่วมทั้งหมดเป็นเพศหญิง (n = 85) อายุเฉลี่ย 58.11 ± 9.72 ปี ยาที่ได้รับบ่อยที่สุดคือ tamoxifen (57.65%) หลังการดำเนินโปรแกรม ค่ามัธยฐานคะแนนความรู้เพิ่มขึ้นอย่างมีนัยสำคัญจาก 4 เป็น 5 (p < 0.05) พบ DRPs ในผู้ป่วยร้อยละ 41.18 โดยอาการข้างเคียงจากยา เช่น อาการร้อนวูบวาบ เป็นปัญหาที่พบบ่อยที่สุด (24.71%) ค่าเฉลี่ย mMPR เท่ากับ 97.36% สูงกว่าเกณฑ์ความร่วมมือที่กำหนดไว้ที่ร้อยละ 80 ระดับความพึงพอใจอยู่ในระดับสูง (ค่าเฉลี่ย 4.65–4.88) และมีการลดค่าใช้จ่ายในการเดินทางเฉลี่ย 496.06 ± 431.52 บาทต่อครั้ง สรุป: การบูรณาการบริการเภสัชกรรมทางไกลร่วมกับการส่งยาทางไปรษณีย์มีความสัมพันธ์กับการเพิ่มขึ้นของความรู้เกี่ยวกับยา ระดับความร่วมมือในการใช้ยาที่สูง ความพึงพอใจของผู้ป่วยในระดับดี และการลดภาระค่าใช้จ่ายด้านการเดินทางในผู้ป่วยมะเร็งเต้านมที่ได้รับยาต้านฮอร์โมน
