บทคัดย่อ
การศึกษาในต่างประเทศยืนยันประสิทธิภาพของการบริบาลทางเภสัชกรรมต่อการรักษาไวรัสตับอักเสบซี แต่ข้อมูลในประเทศไทยยังมีจำกัด วัตถุประสงค์: เปรียบเทียบอัตราการรักษาหายขาดที่ 12 สัปดาห์ (Sustained virological response at 12 weeks: SVR12) ระหว่างกลุ่มบริบาลทางเภสัชกรรมกับกลุ่มควบคุม และประเมินความร่วมมือในการใช้ยา ปัญหาจากการใช้ยา (Drug-related problems: DRPs) และความปลอดภัยเฉพาะในกลุ่มบริบาลทางเภสัชกรรม วิธีดำเนินการวิจัย: การวิจัยเชิงกึ่งทดลองเปรียบเทียบกับกลุ่มควบคุมย้อนหลัง (Quasi-experimental study with historical control group) ในผู้ป่วยไวรัสตับอักเสบซีที่ได้รับการรักษาด้วยยา Direct-acting antivirals (DAAs) ณ คลินิกไวรัสตับอักเสบ โรงพยาบาลมุกดาหาร แบ่งเป็นกลุ่มควบคุมย้อนหลังที่รักษาตามมาตรฐาน (1 สิงหาคม 2566 – 31 กรกฎาคม 2567) และกลุ่มบริบาลทางเภสัชกรรม (1 ตุลาคม 2567 – 30 กันยายน 2568) ซึ่งได้รับการบริบาลทางเภสัชกรรม ประกอบด้วย การซักประวัติการใช้ยา การให้ความรู้ การประเมินความร่วมมือในการใช้ยาด้วยแบบประเมิน Medication Adherence Scale for Thais (MAST) การตรวจสอบอันตรกิริยาระหว่างยา การจัดการปัญหาจากการใช้ยา และการติดตามเหตุการณ์ไม่พึงประสงค์จากยา ผลการวิจัย: กลุ่มบริบาลทางเภสัชกรรม (n = 74) มีอัตราการรักษาหายขาดที่ 12 สัปดาห์ (SVR12) ร้อยละ 98.65 เทียบกับกลุ่มควบคุม (n = 71) ร้อยละ 97.18 ผลต่างความเสี่ยงร้อยละ 1.47 (95% CI: -4.79–8.44) เมื่อปรับปัจจัยเพศและค่าเม็ดเลือดขาวเริ่มต้นด้วย Multivariable logistic regression พบว่า SVR12 ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (adjusted odds ratio = 2.25; 95% CI: 0.18–28.37; p = 0.532) กลุ่มบริบาลทางเภสัชกรรมมีความร่วมมือในการใช้ยา (median MAST score = 40; IQR = 0; range, 35–40) พบ DRPs 15 เหตุการณ์ในผู้ป่วย 14 ราย (ร้อยละ 18.92) โดยข้อเสนอแนะได้รับการยอมรับและแก้ไขสำเร็จทั้งหมด และไม่พบเหตุการณ์ไม่พึงประสงค์ในระดับรุนแรง (Grade 3–5) สรุปผลการวิจัย: อัตราการรักษาหายขาดที่ 12 สัปดาห์ ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่างกลุ่มบริบาลทางเภสัชกรรมและกลุ่มควบคุม ขณะที่กลุ่มบริบาลทางเภสัชกรรมมีความร่วมมือในการใช้ยา ปัญหาจากการใช้ยาได้รับการแก้ไขทั้งหมดและไม่พบเหตุการณ์ไม่พึงประสงค์ระดับรุนแรง อย่างไรก็ตาม ผลลัพธ์ดังกล่าวไม่ได้เปรียบเทียบกับกลุ่มควบคุม
