บทคัดย่อ
การแพทย์ทางไกล (Telemedicine) ได้ถูกนำมาใช้เพื่อเพิ่มประสิทธิภาพในการให้บริการและลดความแออัดของสถานพยาบาลในช่วงสถานการณ์การแพร่ระบาดของโรคโควิด-19 วัตถุประสงค์: เพื่อศึกษาการใช้ระบบแพทย์ทางไกลในการดูแลผู้ป่วยนอกสูงอายุระหว่างการระบาดของโรคโควิด-19 รวมถึงศึกษาปัจจัยที่มีผลต่อการให้บริการแพทย์ทางไกล วิธีดำเนินงานวิจัย: การสัมภาษณ์แบบกึ่งโครงสร้าง (semi-structured interview) บุคลากรทางการแพทย์ 27 คน และผู้รับบริการสูงอายุ 24 คน จากโรงพยาบาลระดับตติยภูมิ 3 แห่ง (รพ.ศิริราช รพ.ศรีนครินทร์ และ รพ.เชียงรายประชานุเคราะห์) ผลการวิจัย: โรงพยาบาลได้พัฒนาการบริการแพทย์ทางไกลเพื่อให้บริการผู้ป่วยนอกรวมถึงผู้ป่วยสูงอายุโรคเรื้อรัง โรคมะเร็ง ผู้ป่วยระยะท้าย ที่มีอาการคงที่ ยินดีรับบริการแพทย์ทางไกล และสามารถใช้เทคโนโลยีเพื่อรับบริการแพทย์ทางไกลได้ ขั้นตอนในการให้บริการแบ่งเป็น 3 ขั้นตอนหลัก ได้แก่ 1) การคัดเลือกและการลงทะเบียนผู้รับบริการ 2) การพบแพทย์ผ่านระบบแพทย์ทางไกล 3) การรับยา โดยปัจจัยที่มีผลต่อการให้บริการแพทย์ทางไกล ได้แก่ กระบวนการให้บริการที่เชื่อมโยงการการบริการเดิม บุคลากรที่ให้บริการ ประสิทธิภาพและความเพียงพอของอุปกรณ์และเทคโนโลยี ความเหมาะสมของสถานที่ให้บริการการเข้าถึงเทคโนโลยีของผู้สูงอายุ การยอมรับของผู้รับบริการ สรุป: การบริการแพทย์ทางไกลมีประโยชน์ในการให้บริการผู้ป่วยสูงอายุเพื่อให้ได้รับบริการอย่างต่อเนื่อง จึงควรพัฒนาการให้บริการดังกล่าวให้มีประสิทธิภาพมากขึ้นในอนาคตเพื่อลดความแออัดของโรงพยาบาลและผู้ป่วยสูงอายุสามารถเข้าถึงบริการได้สะดวก
