บทคัดย่อ
การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลของการให้การบริบาลทางเภสัชกรรมแก่ผู้ป่วยโรคอารมณ์สองขั้วที่ได้รับยาลาโมไตรจีนเป็นครั้งแรก โดยมีผลลัพธ์การศึกษา คือ การเกิดปัญหาที่เกี่ยวกับยาและการเกิดผื่นแพ้ยา วิธีการวิจัย: เป็นการวิจัยเชิงทดลองแบบสุ่มและไปข้างหน้า ของกลุ่มผู้ป่วยนอกโรคอารมณ์สองขั้วที่เข้ารับการรักษาในโรงพยาบาลศรีธัญญาและได้รับการรักษาด้วยยาลาโมไตรจีนเป็นครั้งแรกในช่วงระหว่างวันที่ 5 เมษายน ถึงวันที่ 30 พฤศจิกายน พ.ศ. 2561 แบ่งผู้เข้าร่วมการวิจัยเป็นกลุ่มควบคุม (ได้รับการบริการตามมาตรฐานของโรงพยาบาล) และกลุ่มศึกษา (ได้รับการบริบาลทางเภสัชกรรม) ทำการติดตามในด้านของการเกิดปัญหาเกี่ยวกับยา ทั้งก่อนและหลังได้รับยาลาโมไตรจีนนานประมาณ 8 - 12 สัปดาห์ ผลการวิจัย: มีผู้เข้าร่วมการศึกษา 117 ราย อายุเฉลี่ย ± ส่วนเบี่ยงเบนมาตรฐาน 32.0 ± 9.7 ปี พบว่าการให้การบริบาลทางเภสัชกรรมสามารถลดจำนวนปัญหา “การได้รับยาที่ไม่จำเป็น” (p < 0.001) ระบุถึง “การเกิดอาการไม่พึงประสงค์” ได้มากขึ้น (p = 0.039) และมีค่าเฉลี่ยร้อยละการรับประทานยาเพิ่มขึ้น (p = 0.041) ขณะที่การให้การบริการตามมาตรฐานของโรงพยาบาลสามารถลดจำนวนปัญหา “การได้รับยาที่ไม่จำเป็น” (p = 0.016) การศึกษานี้พบผื่นแพ้ยาจากยาลาโมไตรจีนร้อยละ 11.97 และเกิดขึ้นในระยะเวลาโดยเฉลี่ย ± ส่วนเบี่ยงเบนมาตรฐาน 18.8 ± 17.5 วัน สรุป : การให้การบริบาลทางเภสัชกรรมสามารถลดจำนวนปัญหาการได้รับยาที่ไม่จำเป็น ระบุการเกิดอาการไม่พึงประสงค์ได้มากขึ้นและมีค่าเฉลี่ยร้อยละการรับประทานยาเพิ่มขึ้น สำหรับการเกิดผื่นแพ้ยา เภสัชกรควรที่จะเฝ้าระวังการเกิดผื่นแพ้ยาในผู้ป่วยที่ได้เริ่มใช้ยา Lamotrigine
