ปัจจัยทำนายกระบวนการรับรู้ และคุณภาพชีวิตในผู้ป่วยจิตเภท

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาปัจจัยที่ส่งผลกระทบต่อกระบวนการรับรู้ และคุณภาพชีวิตของผู้ป่วยจิตเภท วิธีการศึกษา: เป็นการศึกษาเชิงวิเคราะห์ กลุ่มตัวอย่างคือผู้ป่วยจิตเภทโรงพยาบาลหนองสองห้อง รวบรวมข้อมูลด้วยเครื่องมือ 3 ประเภท ได้แก่ 1.แบบประเมินการรับรู้ Montreal Cognitive Assessment (MoCA) ฉบับภาษาไทย 2. แบบประเมินคุณภาพชีวิตขององค์การอนามัยโลกชุดย่อฉบับภาษาไทย (WHOQOL–BREF–THAI) 3. แบบประเมินอาการทางจิต Brief Psychiatric Rating Scale (BPRS) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและการวิเคราะห์ถดถอยพหุเชิงเส้น ผลการศึกษา: กลุ่มตัวอย่าง 42 ราย ส่วนใหญ่เป็นเพศชาย 28 ราย (ร้อยละ 66.7) ผู้ป่วยทุกคนมีกระบวนการรับรู้บกพร่อง เนื่องจากมีคะแนน MoCA น้อยกว่า 25 คะแนน โดยมีคะแนนเฉลี่ย 15.38 ± 4.33 คะแนน ส่วนใหญ่มีอาการทางจิตอยู่ในระดับน้อย (ร้อยละ 90.47) และมีคุณภาพชีวิตระดับปานกลาง คะแนนเฉลี่ย 90.62 ± 10.89 คะแนน ปัจจัยทำนายที่มีผลทางลบต่อกระบวนการรับรู้ในผู้ป่วยจิตเภทที่มีนัยสำคัญ ได้แก่ โรคลมชัก เบาหวาน การใช้ยา clozapine (P<0.05) ปัจจัยทำนายที่มีผลทางลบต่อคุณภาพชีวิตในผู้ป่วยจิตเภทที่มีนัยสำคัญ คือ การสูบบุหรี่ (P=0.029) สรุป: การศึกษานี้ได้แสดงให้เห็นถึงผลของปัจจัยต่างๆต่อกระบวนการรับรู้ และคุณภาพชีวิต ดังนั้นการติดตามและควบคุมปัจจัยเหล่านี้จะทำให้ผลลัพธ์การรักษาของผู้ป่วยจิตเภทดีขึ้นได้
