บทคัดย่อ
ตัวชี้วัดและเกณฑ์ที่ใช้ในประเมินการดำเนินงานเภสัชกรรมปฐมภูมิในหน่วยบริการปฐมภูมินั้นถูกกำหนดโดยหลายหน่วยงาน แต่มีข้อจำกัดบางประการทำให้ใช้งานได้ยาก และยังไม่พบหลักฐานการพัฒนาเกณฑ์อย่างเป็นระบบ วัตถุประสงค์: เพื่อพัฒนาตัวชี้วัดและเกณฑ์การประเมินงานเภสัชกรรมปฐมภูมิสำหรับหน่วยบริการปฐมภูมิระดับตำบล วิธีดำเนินการวิจัย: การศึกษานี้ใช้วิธีเดลฟายแบบปรับปรุง ผู้วิจัยพัฒนาตัวชี้วัดและเกณฑ์การประเมินฉบับร่างจากการทบทวนแบบประเมินของหน่วยงานระดับชาติ 3 แห่ง ตัวชี้วัดมีความครอบคลุมงานเภสัชกรรมปฐมภูมิ 4 ด้าน (การพัฒนาระบบยา งานบริการเภสัชกรรมในหน่วยบริการปฐมภูมิ งานบริบาลเภสัชกรรมแก่ผู้ป่วยที่บ้าน และงานคุ้มครองผู้บริโภค) และแบ่งเป็น 3 ประเภท (ตัวชี้วัดปัจจัยนำเข้า ตัวชี้วัดกระบวนการ และตัวชี้วัดผลผลิต) รวมทั้งหมด 25 ตัวชี้วัด แต่ละตัวชี้วัดมีเกณฑ์ประเมิน 4 ระดับ (คะแนน 0, 1, 2, 3) พร้อมคำนิยามของตัวชี้วัดและคำอธิบายของการให้คะแนนแต่ละระดับ จากนั้นส่งให้ผู้เชี่ยวชาญด้านเภสัชกรรมปฐมภูมิจำนวน 14 คน ให้คะแนนความเหมาะสม ด้วยมาตรวัด 5 ระดับ จาก 1 (เหมาะสมน้อยที่สุด) ถึง 5 (เหมาะสมมากที่สุด) วิเคราะห์หาความเห็นพ้องด้วยการใช้ค่ามัธยฐานที่มีค่าตั้งแต่ 3.50 ขึ้นไป และพิสัยควอไทล์ที่ไม่เกิน 1.50 ผลการวิจัย: ตัวชี้วัดและเกณฑ์การประเมินฉบับร่างผ่านการพิจารณาโดยผู้เชี่ยวชาญจำนวนสองรอบ พบว่าผู้เชี่ยวชาญมีความเห็นพ้องต่อตัวชี้วัดและเกณฑ์ส่วนใหญ่ แต่เกณฑ์ประเมินของตัวชี้วัดที่ 5.8 “การมีส่วนร่วมของภาคีเครือข่ายในชุมชนในการแก้ไขปัญหาการคุ้มครองผู้บริโภคในพื้นที่” นั้นพบว่ามีพิสัยควอไทล์เกิน 1.50 และข้อเสนอแนะของผู้เชี่ยวชาญไม่เป็นไปในทิศทางเดียวกัน จึงตัดตัวชี้วัดและเกณฑ์ประเมินข้อนี้ออก เมื่อสิ้นสุดกระบวนการแล้ว พบว่ามีตัวชี้วัดและเกณฑ์ที่เหมาะสมทั้งสิ้น 24 ตัวชี้วัด แบ่งเป็นตัวชี้วัดปัจจัยนำเข้า 9 ตัว ตัวชี้วัดกระบวนการ 12 ตัว และตัวชี้วัดผลผลิต 3 ตัว สรุป: ตัวชี้วัดที่เหมาะสมสำหรับการประเมินงานเภสัชกรรมปฐมภูมิในหน่วยบริการปฐมภูมิระดับตำบลที่ครอบคลุมงานเภสัชกรรมปฐมภูมินั้นมี 24 ตัวชี้วัด และควรนำไปทดลองใช้เพื่อประเมินความเชื่อมั่นต่อไป
