ผลกระทบของนโยบายควบคุมค่าใช้จ่ายด้านยาและเวชภัณฑ์: กรณีศึกษาโรงพยาบาลเวชการุณย์รัศมิ์

บทคัดย่อ
บทนำ: เนื่องจากมูลค่าการใช้ยาและเวชภัณฑ์เพิ่มสูงขึ้นอย่างต่อเนื่อง ในปีงบประมาณ 2560 โรงพยาบาลเวชการุณย์รัศมิ์ จึงมีนโยบายควบคุมค่าใช้จ่ายด้านยาและเวชภัณฑ์ ครอบคลุม 3 ประเด็น คือ 1) นโยบายที่จำกัดด้านราคา 2) นโยบายที่จำกัดปริมาณการใช้ และ 3) นโยบายที่จำกัดปริมาณและราคา การศึกษานี้มีวัตถุประสงค์เพื่อประเมินผลกระทบของนโยบายควบคุมค่าใช้จ่ายด้านยาและเวชภัณฑ์ วิธีดำเนินการวิจัย: การศึกษานี้เป็นการวิจัยเชิงพรรณนา วิเคราะห์ปริมาณและมูลค่าการใช้ยาและเวชภัณฑ์ของโรงพยาบาลก่อนและหลังใช้นโยบาย 1 ปี โดยเก็บข้อมูลก่อนดำเนินนโยบาย ตั้งแต่วันที่ 1 ตุลาคม 2558 จนถึงวันที่ 30 กันยายน 2559 และหลังดำเนินนโยบายตั้งแต่วันที่ 1 ตุลาคม 2559 จนถึงวันที่ 30 กันยายน 2560 ทำการเปรียบเทียบมูลค่าการใช้ยาและเวชภัณฑ์ก่อนและหลังมีนโยบาย ผลการวิจัย: หลังการใช้นโยบายควบคุมค่าใช้จ่ายด้านยาและเวชภัณฑ์ พบว่า มูลค่าการใช้ยาลดลง 15,015,951.75 บาท (ลดลงร้อยละ 11.62) และมูลค่าการใช้เวชภัณฑ์ลดลง 864,627.41 บาท (ลดลงร้อยละ 17.51) โดยมาตรการชะลอการสั่งซื้อยานอกบัญชียาหลักแห่งชาติและให้ใช้ยานอกบัญชียาหลักแห่งชาติหรือยาในบัญชียาหลักแห่งชาติทดแทน สามารถลดมูลค่าการใช้ยามากที่สุดเป็นจำนวนเงิน 2,739,047.75 บาท (ลดลงร้อยละ 40.71) รองลงมา คือ มาตรการตัดรายการยาที่ซ้ำซ้อนในกลุ่มเดียวกัน และมาตรการคัดเลือกยาที่มีชื่อสามัญ (generic name) ในราคาที่ถูกกว่าราคาเดิม สามารถลดมูลค่าการใช้ยาลง 1,862,006.50 บาท (ลดลงร้อยละ 71.68) และ 456,966 บาท (ลดลงร้อยละ 5.12) ตามลำดับ สรุปผลการวิจัย: นโยบายควบคุมค่าใช้จ่ายด้านยาและเวชภัณฑ์ สามารถลดมูลค่าการใช้ยาและเวชภัณฑ์ได้
