บทคัดย่อ
บทนำ: ยาวาร์ฟารินเป็นยาที่สามารถติดตามประสิทธิผลและความปลอดภัยของการรักษาได้จากค่า International normalized ratio (INR) การศึกษานี้มีวัตถุประสงค์เพื่อทราบร้อยละของจำนวนครั้งที่ค่า INR เข้าตามเป้าหมายการรักษาในผู้ป่วยที่ได้รับยาวาร์ฟาริน ณ สถาบันประสาทวิทยา วิธีดำเนินการวิจัย : รูปแบบการศึกษาแบบพรรณนาโดยเก็บข้อมูลย้อนหลังจากเวชระเบียนของผู้ป่วยนอกที่ได้รับยาวาร์ฟารินในทุกข้อบ่งใช้ระยะเวลา 3 ปี (กรกฎาคม พ.ศ. 2552 – กรกฎาคม พ.ศ. 2555) ผลการศึกษา : ผู้ป่วยใช้ยาวาร์ฟารินจำนวน 389 คน อายุเฉลี่ยเท่ากับ 63.04 ปี ผู้ป่วยส่วนใหญ่ได้รับยาวาร์ฟารินเพื่อป้องกันการเกิดภาวะลิ่มเลือดอุดตันในผู้ป่วยที่มีภาวะหัวใจห้องบนเต้นเร็วผิดจังหวะ (ร้อยละ 36.76) ระยะเวลาในการติดตามค่า INR เฉลี่ย 1.05 ปี ความถี่ในการติดตามค่า INR ของผู้ป่วย คือ 10.12 ครั้ง/ปี ร้อยละของจำนวนครั้งที่ค่า INR เข้าตามเป้าหมายการรักษามีเพียงร้อยละ 37.28 โดยพบร้อยละ 47.08 และ 15.64 ของจำนวนครั้งที่ค่า INR ตํ่ากว่าและสูงกว่าเป้าหมายการรักษาตามลำดับ สัดส่วนของผู้ป่วยที่มีความแปรปรวนของระดับ INR ในระดับตํ่า เท่ากับร้อยละ 8.74 กลุ่มยาที่เกิดอันตรกิริยากับยาวาร์ฟารินมากที่สุดคือ ยาลดไขมันในเลือด พบอาการไม่พึงประสงค์จากการใช้ยาวาร์ฟาริน 26 เหตุการณ์ เป็นอุบัติการณ์เกิดภาวะเลือดออกที่รุนแรง 1.95 ครั้ง/100 ปีคน และอุบัติการณ์เกิดภาวะเลือดออกไม่รุนแรง 4.39 ครั้ง/100 ปีคน สรุปผลการวิจัย: ผลจากการศึกษานี้จะนำไปเป็นข้อมูลเพื่อจัดตั้งวาร์ฟารินคลินิกของสถาบันประสาทวิทยาต่อไป
