บทคัดย่อ
ทีโนโฟเวียร์ ไดโซโพรซิล ฟูมาเรต เป็นยาต้านไวรัสในสูตรพื้นฐานตามแนวทางการรักษาและป้องกันการติดเชื้อเอชไอวี แต่มีอาการไม่พึงประสงค์ที่สำคัญคือ การเกิดพิษต่อไต การพัฒนาระบบจัดการความปลอดภัยยาในโรงพยาบาลอาจช่วยป้องกันและลดปัญหาการใช้ยาได้ วัตถุประสงค์: เพื่อพัฒนาระบบการจัดการความปลอดภัยยาทีโนโฟเวียร์ด้วยหลักการลีน การประสานรายการยา และตัวส่งสัญญาณ วิธีการดำเนินการวิจัย: เป็นการวิจัยเชิงปฏิบัติการในคลินิกผู้ป่วยโรคติดเชื้อเอชไอวี ในโรงพยาบาลคลองหลวง โดยใช้วงจรเดมิ่ง PDCA เป็น 4 ขั้นตอน (1) วางแผนปฏิบัติการโดยบุคลากรสหสาขาวิชาชีพ และผู้ให้บริการในคลินิกผู้ป่วยโรคติดเชื้อเอชไอวี จำนวน 12 ราย (2) ปฏิบัติตามแผนจากระยะที่ 1 และเก็บข้อมูลจากตัวชี้วัดที่กำหนด ระหว่างวันที่ 1 ตุลาคม ถึง 30 พฤศจิกายน 2560 จำนวน 189 ราย (3) ติดตามและประเมินผลทุก 1 เดือน จำนวน 2 เดือน ได้แก่ เวลาการให้บริการ จำนวนปัญหาด้านยาและความคลาดเคลื่อนทางยา และความพึงพอใจของผู้ให้บริการ จำนวน 11 ราย (4) กำหนดแนวปฏิบัติที่เหมาะสมเป็นแนวทางการดูแลผู้ติดเชื้อเอชไอวีระบบใหม่ ผลการศึกษาวิจัย: (1) ระบบการจัดการความปลอดภัยยาทีโนโฟเวียร์ด้วยหลักการลีน ลดเวลารอผลตรวจทางห้องปฏิบัติการได้ 16.36 นาที และ 19.86 นาที และลดค่าใช้จ่ายการตรวจที่ไม่จำเป็น 1,470 บาท และ 1,680 บาท ในเดือนที่ 1 และ 2 ตามลำดับ (2) การประสานรายการยา พบ (2.1) การตั้งใจเปลี่ยนรายการยาแต่ไม่ได้บันทึก 1 ราย และการเปลี่ยนแปลงรายการยาที่ไม่ได้ตั้งใจ 2 ราย (2.2) ปัญหาด้านยา 6 ครั้ง (2.3) ความคลาดเคลื่อนทางยา 6 ครั้ง เป็นระดับ B จำนวน 5 ครั้งและระดับ D จำนวน 1 ครั้ง (3) สัญญาณเตือนกำหนดค่าการทำงานของไตที่วิกฤตต้องรายงานด่วน พบ 19 ครั้ง เป็นปัญหาด้านยา 13 ครั้ง ความคลาดเคลื่อนทางยาระดับ B จำนวน 12 ครั้ง ระดับ E จำนวน 1 ครั้ง (4) บุคลากรผู้ปฏิบัติงานมีความพอใจร้อยละ 87.28 เมื่อนำผลการดำเนินงานนำเสนอคณะกรรมการทีมนำทางคลินิกได้รับการอนุมัติเป็นมาตรฐานในการปฏิบัติงานของคลินิกเอชไอวี สรุปผลการวิจัย : การพัฒนาระบบการจัดการความปลอดภัยยา ทีโนโฟเวียร์ ทำให้ลดสิ่งสูญเปล่าที่ไม่จำเป็นในงานบริการ ป้องกันความเสี่ยงจากความผิดพลาดทางยา
