Study of Standard Time and Comparison of Pay for Performance: Case Study in Outpatient Pharmacy Services at Maharat Nakhon Ratchasima Hospital

Abstract
Introduction: Standard time is one of the important variables for Pay for Performance (P4 P) calculation. Objectives: This study has objectives to find standard time in order to evaluate and compare P4P in Outpatient Pharmacy Services at Maharat Nakhon Ratchasima Hospital. Methods: Standard time is calculated by using direct time study method and samples were sampling during October 2014 to April 2015 from pharmacist, pharmacy assistants and pharmacy technicians. The standard time which is calculated from this study will be used to calculate P4P compare with the standard time which is recommended by P4P operation manual. Results: The standard time of outpatient dispensing section, counseling section and inventory management section which is calculated from this study are 4.79 min/prescription, 12.08 min/person and 4.4 min/item respectively. When P4P is taken into consideration by work cycle, the most increasing of P4P is 49,513.86 baht from outpatient dispensing section but the most decreasing of P4P is 25,474.48 baht in inventory management section. For P4P consideration of each person, we can divide 2 cases. The first (the limited payment), we find that P4P of pharmacist, pharmacy assistants and pharmacy technicians have the difference (P4P is both decreased and increased). The second (unlimited payment), P4 P of almost all pharmacist is increased 803.33 baht per month approximately. Conclusion: The study indicates that there is the difference of the before and after standard time which result in P4P.
