Abstract

             This study employed a mixed methods approach with the aim of examining the state of the care model for patients in the early stages of chronic obstructive pulmonary disease (COPD) at Thanyaburi Hospital. The conceptual framework utilized for this study was based on Donabedian's (2003) service quality assessment, encompassing the dimensions of structure, process, and results. Data collection involved a focus group discussion with seven healthcare providers, as well as individual discussions with 10 patients and their relatives. Additionally, a self-management assessment was conducted with twenty COPD patients. Descriptive statistics were applied for quantitative data analysis, while content analysis was employed for qualitative data.

             The findings indicated that the guidelines for the structural aspects of the caring model for COPD patients were intangible and inconsistent. Most healthcare teams provided routine and functional care based on their prior experience in patient care. Continuous monitoring and evaluation of care were inconsistent. The patient care process was general and lacked specificity in terms of empowering patients for enhanced self-management. Outcome evaluation primarily focused on physical assessments and did not prioritize self-management behavior. Consequently, there is a need for a clearly defined care model for early-stage COPD patients, emphasizing the promotion of patient empowerment for self-management. Outcome evaluations of the behavior aspect should encompass both self-management behavior and physical capacities, assisting COPD patients to be able to manage current symptoms, slow the progression of the disease, and improve their overall quality of life.
