ปัจจัยทำนายคุณภาพชีวิตของผู้ป่วยสูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียม

การผ่าตัดเปลี่ยนข้อเข่าเทียมเป็นการรักษาที่พบมากในกลุ่มผู้สูงอายุที่มีปัญหาเรื่องข้อเข่าเสื่อมรุนแรง การประเมินคุณภาพชีวิตในผู้ป่วยหลังได้รับการผ่าตัดจึงเป็นสิ่งสำคัญ การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษา ความสามารถของความปวด ความสามารถในการทำกิจกรรม และภาวะซึมเศร้า ในการทำนายคุณภาพชีวิตของผู้ป่วยสูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียม 6 สัปดาห์ ตัวอย่างคือผู้สูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียมจำนวน 100 ราย ที่มารับบริการโรงพยาบาลตติยภูมิแห่งหนึ่งในกรุงเทพมหานคร มีคุณสมบัติตามเกณฑ์ที่กำหนด เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินความปวด แบบประเมินดัชนีบาร์เทลเอดีแอล แบบวัดความเศร้าในผู้สูงอายุไทย-15 และแบบสอบถามคุณภาพชีวิตโรคสะโพกและข้อเข่าเสื่อม วิเคราะห์ข้อมูลด้วยสถิติบรรยาย สัมประสิทธิ์สหสัมพันธ์เพียร์สัน และการวิเคราะห์ถดถอยพหุคูณ
ผลการศึกษาพบว่า ความปวดมีความสัมพันธ์ทางลบกับคุณภาพชีวิตในระดับสูง (r = .621, p < .01) ในขณะที่ความสามารถในการทำกิจกรรมมีความสัมพันธ์ทางบวกกับคุณภาพชีวิตในระดับปานกลาง (r = .333, p < .01) ความปวด ความสามารถในการทำกิจกรรม ภาวะซึมเศร้า ร่วมกันอธิบายคุณภาพชีวิตในผู้ป่วยสูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียม 6 สัปดาห์ได้ร้อยละ 46 อย่างมีนัยสำคัญทางสถิติ โดยความปวดเป็นปัจจัยที่มีความสามารถในการทำนายสูงสุด ข้อเสนอแนะจากการศึกษาครั้งนี้คือ ควรจัดโปรแกรมติดตามประเมินและจัดการความปวด จัดกิจกรรมเพื่อส่งเสริมความสามารถในการทำกิจกรรมให้กับผู้สูงอายุหลังผ่าตัดเปลี่ยนข้อเข่าเทียมในช่วงหลังจำหน่ายออกจากโรงพยาบาลและนำไปสู่การมีคุณภาพชีวิตที่ดีของผู้สูงอายุต่อไป
