การพัฒนาสมรรถนะพยาบาลหัวหน้าทีมในการดูแลผู้ป่วย septic shock ในห้องฉุกเฉิน

บทคัดย่อ

The research and development study aimed to develop and study the results of developing the competency of leader nurses in caring for septic shock patients in the emergency room. The leader’s nurses have a role in caring for and monitoring the treatment results of patients, evaluating symptoms, reporting to doctors for diagnosis, and providing initial assistance. If nursing care can be provided quickly according to standards, it will help reduce complications and mortality. The study applied the systems theory, consisting of the planning phase, the implementation phase, the observation phase, and the reflection phase. The tools consisted of 1) Practice Guidelines Sepsis, 2) the competency assessment (OSCE), and the indicator data form. The comparative data were analyzed using, mean, standard deviation, and t-test. The results of the study found that 1) after developing the competence of the leader’s nurses, the score was 91.8 percent, and 2) the average diagnosis time was 7.3 minutes, the average blood sample collection time was 15.9 minutes, and the average ATB time was 23.2 minutes, which was faster than the previous year. The number of patients admitted to the ICU increased by 52.2 percent.

The development of a leader’s nurse competency results in good outcomes for patients, the leader’s nurse, and the organization. There are standard nursing practices. Competency development should be expanded to all nurses and patient care outcomes should be assessed to cover all dimensions more to reflect the impact of improving the quality of care for patients with this condition.
