รูปแบบการจัดบริการการดูแลสุขภาพผู้สูงอายุโดยการมีส่วนร่วมของชุมชน ตำบลวังตะกู จังหวัดนครปฐม* Health Care Service Model for Elderly by Community Participation, Wangtagoo Subdistrict, Nakhon Pathom Province

Abstract

การวิจัยครั้งนี้เป็นการวิจัยเชิงปฏิบัติการอย่างมีส่วนร่วม (participatory action research) มีวัตถุประสงค์เพื่อพัฒนารูปแบบการจัดบริการการดูแลสุขภาพผู้สูงอายุโดยการมีส่วนร่วมของชุมชนตำบลวังตะกู จังหวัดนครปฐม กลุ่มตัวอย่างประกอบด้วย ผู้สูงอายุและญาติจำนวน 30 คน ทีมสุขภาพและตัวแทนจากองค์กรในชุมชน จำนวน 15 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ 1) แนวคำถามการสัมภาษณ์เชิงลึก การสนทนากลุ่มและการประชุมแบบมีส่วนร่วม 2) แบบสอบถามประเมินความพึงพอใจของผู้สูงอายุต่อรูปแบบการจัดบริการ การดูแลสุขภาพผู้สูงอายุฯ พัฒนารูปแบบรูปแบบการจัดบริการการดูแลสุขภาพผู้สูงอายุ โดยการมีส่วนร่วมของชุมชน ระหว่างเดือนตุลาคม-กันยายน พ.ศ. 2557 ผลการวิจัย พบว่า 1. ปัญหาการดูแลสุขภาพของผู้สูงอายุ คือ ขาดความรู้ในการรับประทานยา มีความเชื่อในการใช้ยาไม่ถูกต้อง ญาติผู้ดูแลขาดความรู้ในการประกอบอาหารให้กับผู้สูงอายุ และผู้สูงอายุไม่สามารถเข้าถึงบริการทางการแพทย์ได้สะดวกเนื่องจากไม่มีญาติพาไป 2. ความต้องการได้รับการบริการสุขภาพของผู้สูงอายุ คือ การมีบริการการตรวจรักษาที่บ้าน การมีผู้ช่วยเหลือพาเข้าห้องน้ำ พาเดินไปตรวจที่โรงพยาบาล การได้รับความรู้เรื่องผลข้างเคียงของยา การออกกำลังกาย การให้มีแพทย์ประจำมารักษาที่โรงพยาบาลส่งเสริมสุขภาพตำบล และการมีองค์กรบริหารส่วนตำบลวังตะกูมาเยี่ยมผู้สูงอายุที่มีภาวะพึ่งพาผู้อื่นและให้มีพระมาโปรดที่บ้าน 3. รูปแบบการจัดบริการดูแลสุขภาพผู้สูงอายุโดยการมีส่วนร่วมของชุมชน ตำบลวังตะกู จังหวัดนครปฐม ประกอบด้วยกิจกรรม ดังนี้ 1) การสร้างการเข้าถึงบริการโดยการจัดทำสติกเกอร์เบอร์โทรศัพท์ที่สำคัญแจกให้กับผู้สูงอายุ 2) การเยี่ยมบ้านผู้สูงอายุโดยมีส่วนร่วมขององค์ในชุมชน 3) ให้ความรู้กับผู้สูงอายุ ในเรื่องการใช้ยา การออกำลังกาย และการรับประทานอาหาร 4) การดูแลผู้สูงอายุในขณะมารับการรักษาที่โรงพยาบาล โดยชมรมผู้สูงอายุได้จัดจิตอาสามาช่วยดูแลผู้สูงอายุระหว่างมารับการรักษาที่โรงพยาบาล ผลการประเมินพบว่าผู้สูงอายุมีความพึงพอใจต่อรูปแบบการจัดบริการการดูแลสุขภาพที่พัฒนาขึ้น ในภาพรวมอยู่ในระดับมากที่สุด (ร้อยละเฉลี่ย 96.67)

The research is participatory action research. The objective is to develop Health Care Service Model for Elderly by Community Participation, Wamgtagoo Subdistrict, Nakhon Pathom Province. The samples consisted of 30 samples from elderly and relatives and 15 samples from health team and community organization representatives. The data collection tools were 1) questions for indepth interview, focus group discussion and participatory meeting 2) questionnaire for elderly satisfaction of the Health Care Service Model for Elderly. The Health Care Service Model for Elderly by Community Participation was developed during September to October 2014. The research results showed: 1. The problems of health care for the elderly were inadequate knowledge of taking drugs, incorrect knowledge of drug use, inappropriate cooking knowledge of the relatives and inconvenience to health care service access due to unavailability of relatives. 2. Needs for health care services of the elderly were home health care, assistance for going to toilet, assistance during hospital visit, information of drug adverse reaction and exercise, need to have doctors on duty at Tambon Health Promoting Hospitals, need of home visit for dependent elderly from Tambon Administration Organization and need to have a monk pray at their homes 3. The Health Care Service Model for Elderly by Community Participation, Wamgtagoo Subdistrict, Nakhon Pathom Province consisted of 1) distribution of stickers with important telephone numbers to the elderly to increase health service access 2) elderly home visit with community organization participation 3) education of drug use, exercise and diet for the elderly 4) volunteers from the elderly club to take care of the elderly during hospital visit. The satisfaction evaluation showed that the elderly were satisfied with the developed health care service model at highest score (average 96.67%).
