Abstract
Health promotion for stroke patients towards the sustainable development goals arose from the systematic thinking and the mechanism application to
increase caregiver ability by focusing on 5 Ottawa Charter and the potential development of problem solving using community capacity, the ability to search
for social capital and community capacity, the potential to create and develop the analytical team to use empirical data, and the ability to manage the support
system. This health promotion also operated with the caregiving network and the families based on capital and capacity in the community. The prototype
support system for families with stroke patients was implemented and disseminated to local administrative organizations, government, and public sector.
Engaging these agencies to implement the system would lead to a better quality
of life.
