The Development of a Diabetic Care System in Kuchan Health Promoting Hospital, Kham Khuean Kaeo District, Yasothon Province

Abstract
This study is action research. Aimed at developing the diabetes care system in a community under responsibility of the Kuchan Health Promoting Hospital, Kham Khuean Kaeo District Yasothon Province. The study collected all data from study group of 47 persons ,by using the participation of community for managements and applied concept of empowerment for study, Use quantitative data and quantitative data. The data were analyzed by using descriptive statistics, including frequency, percentage, mean and standard deviation. Inferential statistics such as a paired sample t-test was also applied. And quantitative data analysed by content analysis. Results showed that the development of a diabetic care system in Kuchan health promoting hospital , Kham Khuean Kaeo district , Yasothon province has got the process are as follow. 1) Study the basic data and each context of any area before preparing of meeting. 2) Evaluate the knowledge action and participationbefore processing. (pre-test) 3) Set the meeting with all of the participants to analyze the situation. 4) Meeting to make the action plan by using the participation guide
line and empowerment idea. 5) Action as the plan. (being for action) 6) supervision, evaluation of procession , observe and interview 7) Evaluation of knowledge , action participation and satisfaction after procession (post –test) . 8) Set the stage to exchange knowledge for reflection. 9) Conclude the factors of successful. 10) Problem and obstacle, suggestion and the way to solve the problem which the procession can help the populations learn to know more increased of diabetic and the significantly comparative to those prior to operational period, data was significant
at p< 0.05. Process of development is "Knowledge Unity Community Participation Healthy Appreciate Network" or KUCHAN Model. From them use the system had findings as follows: Diabetes Patients gained more knowledge, practices, support and satisfaction data was significant at p< 0.05.
