Cardiovascular Risk Factors in Young Adult Patients with Acute Ischemic Stroke

Abstract

Background: The number of acute ischemic stroke in young adults (age 45 years and less) has increased steadily. It is not well established whether young stroke patients had different risk factors as found in adults aged more than 45 years. Objective: The purpose of this study was to compare risk factors of acute ischemic stroke in young adults and those aged more than 45 years. Methods: We analyzed data from the stroke registry of Hat Yai Hospital from October 2011 to February 2016. There were 6 major risks factors including hypertension, diabetes, hyperlipidemia, previous stroke, atrial fibrillation, and previous cardiovascular diseases. Two minor risk factors including of smoking, and alcohol drinking. We compared individual risk factors using Chi-square, odds ratios (OR), and 95% confidence interval (95%CI). Results: There were 3,133 patients with a final diagnosis of acute ischemic stroke, aged 16-85 years (mean 63.7±14.2 years), 1,874 men (59.8%). Young adults were 323 (10.3%) cases. Smoking (OR=1.36, 95%CI 1.08-1.71, p=0.010), and alcohol used (OR=2.08, 95%CI 1.65-2.63, p<0.001) were associated with acute ischemic stroke in young adult patients. They had less diabetes (OR=0.59, 95%CI 0.44-0.81, p=0.001), hypertension (OR=0.39, 95%CI 0.30-0.49, p<0.001), dyslipidemia (OR=0.46, 95%CI 0.33-0.64, p<0.001), and previous history of stroke (OR=0.64, 95%CI 0.41-0.98, p=0.038). Conclusion: Young adults stroke had higher risk of smoking and alcohol used than did adults aged more than 45 years. Primary and secondary stroke prevention in young adults should be more considered in modification smoking and alcohol consumption.
