Critical View of Safety: A Safe Method to Prevent Bile Duct Injury from Laparoscopic Cholecystectomy

The incidence of bile duct injury after laparoscopic cholecystectomy is 0.20 - 3.24%. This may cause post-operative morbidity to the patients. Factors associated with bile duct injury included training experience, disease severity, anomalous anatomy and technical errors. The “Critical view of safety” is a technique for dissection of the Calot’s triangle by clearing fatty and fibrous tissue for identification of cystic duct and cystic artery, which were directed into the gall bladder. This is a very important step before clipping and ligation of cystic duct and cystic artery for preventing bile duct injury. However, in case of the cleared anatomy was not visualized, intra-operative cholangiography or conversion to open cholecystectomy should be considered for patient safety.
วิธีการผ่าตัดถุงน้ำดีที่ปลอดภัยโดยวิธีส่องกล้อง เพื่อป้องกันการเกิดภยันตรายต่อท่อทางเดินน้ำดี
อุบัติการณ์ของการเกิดภยันตรายต่อท่อทางเดินน้ำดีภายหลังการผ่าตัด laparoscopic cholecystectomy พบได้ ประมาณ 0.20-3.40% ซึ่งภาวะเเทรกซ้อนนี้อาจส่งผลให้เกิดภาวะทุพพลภาพกับผู้ป่วยได้ปัจจัยส่งผลให้เกิดภยันตรายต่อท่อ ทางเดินน้ำดีได้แก่ training experience, disease severity, anomalous anatomy และ technical errors การผ่าตัด laparoscopic cholecystectomy โดยใช้เทคนิค “Critical view of safety” เป็นการผ่าตัดเลาะเนื้อเยื่อบริเวณ Calot’s triangle ให้เห็นลักษณะกายวิภาคของโครงสร้าง 2 โครงสร้างที่วิ่งตรงเข้าหา gall bladder คือ cystic duct และ cystic arteryก่อนทำการหนีบและตัดจะช่วยลดอุบัติการณ์ของการเกิดภยันตรายต่อท่อทางเดินน้ำดี อย่างไรก็ตาม หากไม่สามารถ เห็นกายวิภาคของโครงสร้างดังกล่าวข้างต้นได้อย่างชัดเจน การใช้intraoperative cholangiography หรือ การพิจารณา เปลี่ยนเป็นการผ่าตัดแบบเปิด (open cholecystectomy) อาจช่วยให้มีความปลอดภัยต่อผู้ป่วยมากขึ้น
