มะเร็งตับอ่อนชนิดที่พบได้น้อย Carcinosarcoma: รายงานผู้ป่วยและการทบทวนวรรณกรรม

บทคัดย่อ
กรณีศึกษาผู้ป่วยหญิงไทย อายุ 67 ปี ปฏิเสธโรคประจำตัว มาด้วยอาการจุกแน่นบริเวณลิ้นปี่ ตรวจร่างกายพบว่า คลำได้ก้อนบริเวณช่องท้องด้านซ้ายส่วนบน ประมาณ 10 เซนติเมตร ผลการทำอัลตราซาวน์ช่องท้องพบก้อนถุงน้ำขนาดใหญ่บริเวณช่องท้องบนด้านซ้ายติดกับม้ามและไต หลังจากนั้นส่งทำเอกซเรย์คอมพิวเตอร์ภาพตัดขวางพบว่าเป็นก้อนถุงน้ำขนาดใหญ่และมีผนังกั้นเกิดจากส่วนหางของตับอ่อน ร่วมกับม้ามขาดเลือดไปเลี้ยง ผู้ป่วยจึงได้รับการวินิจฉัยว่าเป็นเนื้องอกมะเร็งตับอ่อนชนิดถุงน้ำ ผู้ป่วยได้รับการผ่าตัดเอาก้อนตับอ่อนส่วนปลายและม้ามออก ผลทางพยาธิวิทยาพบว่าเป็นชนิด Carcinosarcoma ซึ่งเป็น Biphasic tumors มีส่วนประกอบของ Malignant epithelial และ Spindle cells หลังจากผ่าตัดประมาณ 1 ปี ผู้ป่วยมีอาการไอเรื้อรัง ภาพเอกซเรย์ปอดพบก้อนกระจายไปที่ปอดทั้งสองข้าง เอกซเรย์คอมพิวเตอร์ภาพตัดขวางพบมีก้อนเกิดขึ้นใหม่ที่ตับอ่อนร่วมกับมีลุกลามไปที่ไตข้างซ้าย และมีการกระจายไปที่ตับและปอด
