Abstract

The objectives of this research are as follows: (1) to study the characteristics of knee replacement surgery patients; and (2) to compare cost, length of stay and effectiveness in quality of care between one-sided bilateral surgery in total knee replacement patients. This study was collected by a retrospective review of medical records in the Golden Jubilee Medical Center, Thailand, during October 2020-September 2021. The samples in this study were 152 patients by specific selection. The data were analyzed by descriptive statistics, Chi-square test, Fisher’s exact test and an independent t-test. The results of the study were as follows: (1) most of the total knee replacement patients in the Golden Jubilee Medical Center were elderly women who had one-sided surgery in total knee replacement. They are special patients and used in the general department of comptrollers, the average length of stay was 5.91 days, the average treatment costs were 152,178.57 baht (33,618.50 - 261,725.25 baht) and all of them were improved status when discharged; (2) there was no significant difference in length of stay, beyond the seven-day treatment plan, complications, pain symptoms, and ambulation with walker, Oxford knee scores at three months, six months, and 12 months and patient satisfaction. Except for the average cost, the one-sided surgery group was significantly less than the bilateral surgery group at 0.05 level of significance. If the patient has to have surgery on both sides, and choosing bilateral total knee replacement surgery will be more cost-effective.
