Abstract
A 67-year-old Thai woman with no underlying disease presented with epigastrium pain and a 10-cm palpable mass in the left upper abdomen on physical examination. Ultrasonography showed a solid-cystic mass on the left side of the abdomen that abutted the spleen and left kidney. Multiphase computed tomography presented a large lobulated mixed cystic/solid mass with internal, multiple and thick enhancing septa and enhancing a solid portion of the pancreatic tail, associated with splenic infarction. The patient was diagnosed with a pancreatic mucinous cystic neoplasm and underwent distal pancreatectomy with a splenectomy. A histopathologic examination revealed a carcinosarcoma and biphasic tumors consisting of malignant epithelial and spindle cells. One year after surgery, the patient presented with a chronic cough. The chest radiography revealed multiple lung metastasis. The computed tomography confirmed a recurrent tumor at distal pancreas with left renal invasion, as well as liver and lung metastasis.
