บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลการดำเนินงานอุปสรรค และข้อเสนอแนะของระบบการดูแลระยะ ยาวเก็บข้อมูลวิธีเชิงปริมาณในผู้สูงอายุที่มีภาวะพึ่งพิงจากการประเมินความสามารถในการทำกิจวัตรประจำวัน 11 และมีแผนการดูแล 195 คน เครื่องมือวิจัยประกอบด้วยแบบประเมินความสามารถในการทำกิจวัตรประจำ วันและการประเมินด้วยภาพประกอบ แบบประเมินผลตามตัวชี้วัดของสำนักงานหลักประกันสุขภาพแห่งชาติ ผลการดำเนินงานและแบบสอบถามความพึงพอใจของญาติผู้ดูแล/ผู้สูงอายุต่อระบบการดูแลระยะยาววิเคราะห์ ข้อมูลด้วยความถี่ร้อยละค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน เก็บข้อมูลวิธีเชิงคุณภาพจากอนุกรรมการสนับสนุน การจัดบริการดูแลระยะยาวและหรือผู้จัดการระบบดูแลระยะยาว 14 คน ผู้ช่วยเหลือดูแลผู้สูงอายุ 10 คน ญาติผู้ดูแล/ผู้สูงอายุ 5 คนโดยสัมภาษณ์เชิงลึกและสนทนากลุ่ม วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา ผลการศึกษาพบว่าผลการดำเนินงานระบบการดูแลระยะยาวอยู่ในระดับมาก ( = 3.88, S.D. = 0.45) ผลประเมินตามตัวชี้วัดของสำนักงานหลักประกันสุขภาพแห่งชาติพบว่า ไม่มีศูนย์พัฒนาคุณภาพชีวิตผู้สูงอายุใน ชุมชน กลุ่มตัวอย่างมีความพึงพอใจในระดับมาก ( = 4.14, S.D. = 0.39) ระบบการดูแลระยะยาวสามารถลด การเกิดภาวะแทรกซ้อนจากแผลกดทับและทำให้ผู้สูงอายุมีสุขภาพดีขึ้น อุปสรรคในการดำเนินงาน คือ 1) ระเบียบและขั้นตอนการเบิกจ่ายยุ่งยากไม่ชัดเจน 2) ผู้จัดการระบบต้องปฏิบัติงานจัดซื้อจัดจ้าง ซึ่งไม่ถนัด 3) ญาติผู้ดูแล บางรายไม่ให้ความร่วมมือในการเยี่ยมผู้สูงอายุ 4) TAI ไม่สอดคล้องกับการเบิกจ่าย ข้อเสนอแนะจากผู้ให้ ข้อมูลหลัก 1) ปรับระบบงานให้คล่องตัว 2) กำหนดชื่อวัสดุที่สามารถเบิกจ่ายให้ชัดเจน 3) ท้องถิ่นควรดำเนินงาน จัดซื้อจัดจ้างและตั้งคลังวัสดุทางการแพทย์ของการดูแลระยะยาว 4) พิจารณาเลือกเฉพาะผู้สูงอายุที่จำเป็น ต้องได้รับการช่วยเหลือ 5) ผู้ช่วยเหลือได้รับการทำประกันอุบัติเหตุ 6) ฟื้นฟูความรู้และทักษะ ผู้ช่วยเหลือดูแล อย่างต่อเนื่อง
