Effects of Dolutegravir-based Antiretroviral Therapy on Serum Creatinine eGFR and Risk Factors Influencing Renal Impairment in Patients living with HIV Namom Hostital: Retrospective Descriptive Study Namom Hostital.

Abstract

Background The care and treatment of people living with HIV (PLWH) with anti-HIV drugs has continually evolving. Currently, antiretroviral drugs are being developed that contain the drug Dolutegravir, which can lead to kidney damage.

Objective To study the effect of using antiretroviral drugs containing Dolutegravir in the drug regimen on Cr and eGFR levels in PLWH. Before and after taking antiretroviral drugs that contain dolutegravir.

Materials and Methods This retrospective descriptive study among 53 PLWH at ARV Clinic, Namom Hospital between 2022 - 2024. Research Instrument is an PLWH information form.  Data were analyzed using descriptive statistics,Chi-square, and logistic test.
Results The results revealed a significant decline in renal function post-treatment, with the proportion of PLWH having an eGFR <60 mL/min/1.73 m² rising from 3.8% - 15.1%. The individual eGFR changes ranged from -3.01 - 43.74, with an average decrease of 17.81% per person, representing a statistically significant difference (p< .001). Correspondingly, the prevalence of abnormal Serum Cr levels increased from 5.7% ( = .82 S.D.= .20) - 17.0%   ( = 1.02 S.D.= .25) after therapy initiation (p< .001). Notably, aged over 60 years were at the highest risk, with a 7-fold greater likelihood of experiencing a significant eGFR decline compared to other age groups (95%CI: 1.46–39.61).

Conclusions   Antiretroviral drugs containing dolutegravir has an effect on kidney function, especially in older PLWH. PLWH should be evaluated continuously after administration.
