Factors Associated with 24-hour Mortality Among Emergency Department Patients with Septic Shock at Maharaj Nakhon Si Thammarat Hospital

Abstract

Background: Septic shock is a life-threatening emergency with a high mortality rate, particularly within the first 24 hours, reflecting both disease severity and the effectiveness of early management. However, data on factors associated with early mortality in these patients remain limited.

Objective: To identify factors associated with 24-hour mortality among patients with septic shock presenting to the emergency department at Maharaj Nakhon Si Thammarat Hospital

Materials and Methods: This retrospective cohort study included patients aged ≥15 years diagnosed with septic shock (ICD-10: R57.2) or enrolled in the sepsis fast-track system who were treated in the emergency department between February 1 and December 31, 2023. Data were collected from hospital databases and electronic medical records. Descriptive statistics, Mann-Whitney U test, Chi-square test, and multivariable logistic regression were performed.

Results: A total of 615 patients were included; most were male, with a median age of 67 years (IQR 54-77). The 24-hour mortality rate was 16.7%, and the overall mortality rate was 45%. Of all patients, 35.6% were admitted to the intensive care unit. Common clinical features included respiratory failure, vasopressor use, and elevated initial serum lactate levels. Respiratory tract infection was the most common source of infection. Factors significantly associated with 24-hour mortality were elevated initial lactate levels (adjusted OR 1.23, 95% CI 1.15–1.32) and presence of respiratory failure (adjusted OR 2.77, 95% CI 1.22–7.15).

Conclusions: Elevated initial lactate levels and presence of respiratory failure are associated with 24-hour mortality among patients with septic shock.
