Abstract

Introduction: Medication knowledge conforming to the problems of the organization was the important key that encourage the organization to realize the significance of the medication error (ME) report. Likewise, it emphasized the actual problems to provide the best practice guideline for patient safety regarding medication use under the collaboration of the healthcare team (HCT).

 Objectives: To encourage the in-patient ME reports, to create medication knowledge for the best practice in-patient care and to study the overall satisfaction of pharmacy officers and HCT in Pediatrics and Surgical wards.

 Materials and Methods: We analyzed nurse ME report that sent to pharmacists who worked at in-patient dispensing unit and used questionnaires to compare HCT’s satisfaction before and after knowledge management (KM) implementation (October-December 2018 and January-March 2019) respectively in order to create medication knowledge for sharing in HCT.

Results: After implementation of KM, we found that 1) the in-patient ME reports were increased from 294 to 600 reports, 2)mmjmทedication knowledge was created before and after implementation of KM as 103 and 62 topics, respectively, 3) the overall satisfaction of HCT was increased from 86.26% to 100%. 

Conclusion: KM in medication was the appropriate strategy to encourage the in-patient ME reports resulting in HCT’s satisfaction improved because they could practically apply KM which conformed to the problems of the organization.
