Removal of Silver Cones and Broken Instruments from Maxillary First Molar: A case report

Abstract

Root canal treatment usually fails when treatment falls short of acceptable standards. The reason is because of procedural error such as broken instrument that prevent the control and prevention of intracanal endodontic infection. An instrument fragment in itself is rarely the direct cause of problem; it does, however, limit access to the apical part of the canal, compromising disinfection and obturation. The clinical situation (existence of periapical lesion), stage of canal preparation, fracture position, and type of fractured instrument can significantly influence prognosis and the approach to management. This case reports removal of 3 pieces of silver cones and broken instruments from maxillary first molar using a combination of hand instrumentation and ultrasonic devices. The 1-year follow-up demonstrates reduction of periapical lesion and no pathologic changes of clinical examination. This case report present a treatment strategy that improves the healing outcomes.
