Service system of COPD Clinic of ChianYai Hospital, Nakhon Si Thammarat

Abstract

Objective: The purposes of this research are (1) to study service management of COPD clinic of ChianYai Hospital, Nakhon Si Thammarat, and(2) to investigate knowledge and behaviors that resist the exacerbation of COPD patients of ChianYai Hospital, Nakhon Si Thammarat.

 

Materials and Methods: The research uses qualitative method. The research started from September 18th to October 2nd 2017. The sample used are (1)10 medical personnels who take care of COPD patients, and (2) 16 COPD patients, 18 years old and above, who take treatment at COPD clinic of ChianYai Hospital, Nakhon Si Thammarat during September 18th to October 2nd 2017, chosen using purposive sampling. The instruments used include: (1) the interview form of medical personnels and patients; and (2) the observation the bronchodilator reliever usage  demonstration and medicine taking of patients.

 

Result:  The findings reveal that ChianYai Hospital, Nakhon Si Thammarat provides methods of treatment and directions of preventing COPD exacerbation patientsbut the hospital does not take those methods and directions in typically practice. The hospital provides academic training and assigns responsibility roles for each sector. The information is reviewed annuallyand has reached all criteria but the statistics show that COPD patients is the largest number taking treatment asIPD and was the 6th ranking of emergency patient in 2016. There is COPD clinic , Fahsai clinic (stop smoking clinic), emergency room of COPD exacerbation patients, and also home visiting units. The study of knowledge and behaviors show that COPD patients know about causes and prevention of COPD exacerbation and behave in directions to prevent the disease but there are social activities that effect COPD exacerbation i.e. agricultural burning, mosquito incense. The พ่นยา demonstration showed that 10 patients did wrong process of Bronchodilator usage and did not bring reliever bronchodilator device with them.
