Abstract

Introduction: Stroke is a common emergency conditionthat has been increasing and causing deaths. The survivors from this disease will have disabilities and will be resulted in a poor quality of life. Patients who have fast detection of symptomswill have a better chance of receiving treatment or medication. Finally, it can reducethe rate of death or disability.

 Objective: The purpose of this study was to evaluate the process and the result of caring stroke patients in acute stage at Emergency department, Maharaj Nakhon Si ThammaratHospital.

 Materials and Methods: Across-sectional descriptive study was conducted in acute stroke patients. A total 170 patients were selected by purposing sampling technique who were used emergency unit during October to December 2015, and register nurse who cares each patient. The instrument used for data collection is questionnaire and consist of 3 parts; 1) General information 2) Recordingform of nursing practice guidelines and nursing practice outcomes 3) Recording form of the patient outcomes in acute stage at Emergency department. The statistical analysis was descriptive design.

 Result: The results revealed that 44.11 percent of stroke patient were in acute stage in fast tract category. The average age was 66.30 years old. Most of them had hypertension and dyslipidemia, 75.88 percent was delivered from community hospital. The conclusion of process found that most of nursing activities record was missing especially the surveillance of complication and clinical risk as 33.58 percent. For the treatment outcome, the time for delivery the stroke patient in fast track to computed tomography of brain is 28.12 minutes in average.

 Conclusion: From the result of this study, the stroke patient care system should be developed and cover general population, risk patient, emergency medical system and hospital care. So,if the patients got medical care quickly, can reducemortality and disability rate.
