Impact of Different Fluoride Treatments on Oral Health-Related Quality of Life of Primary School Children

Abstract

คุณภาพชีวิตในมิติสุขภาพเป็นสิ่งสำคัญในการประเมินผลการรักษาในมุมมองของผู้ป่วย วัตถุประสงค์การศึกษานี้เพื่อประเมินคุณภาพชีวิตในมิติสุขภาพช่องปากในเด็กจากผลการรักษาด้วยฟลูออไรด์ 3 วิธี เป็นการศึกษาทางคลินิกแบบสุ่ม อาสาสมัครเป็นนักเรียนอายุ 6-7 ปี ที่มีฟันกรามน้ำนมผุในชั้นเนื้อฟันชนิดลุกลามอย่างน้อย 1 ซี่ จำนวน 253 คน แบ่งเป็น 3 กลุ่ม กลุ่ม 1 ซิลเวอร์ไดเอมีนฟลูออไรด์ร้อยละ 38 กลุ่ม 2 โซเดียมฟลูออไรด์วาร์นิช ร้อยละ 5 และกลุ่ม 3 ใช้ร่วมกันทั้ง 2 ชนิด เด็กได้รับการตรวจฟัน และทาฟลูออไรด์ทุก 6 เดือน สัมภาษณ์เด็กด้วยดัชนีคุณภาพชีวิตในมิติสุขภาพช่องปากสำหรับเด็ก ณ เวลา 1 ปี พบว่าลักษณะประชากร และสภาวะทางช่องปากของทั้ง 3 กลุ่ม แตกต่างกันอย่างไม่มีนัยสำคัญ พบฟันที่มีสีดำในกลุ่มที่ได้รับซิลเวอร์ไดเอมีนฟลูออไรด์ทั้ง 2 กลุ่ม มากกว่ากลุ่มที่ไม่ได้รับอย่างมีนัยสำคัญทางสถิติ (p<0.001) โดยความชุกของผลกระทบสุขภาพช่องปากต่อชีวิตประจำวันของเด็กแตกต่างกันอย่างไม่มีนัยสำคัญทางสถิติ (p=0.64) ส่วนคะแนนผลกระทบในด้านการรับประทานอาหาร การยิ้ม หัวเราะ ในทุกกลุ่มสูงกว่าด้านอื่นๆ แต่ไม่มีนัยสำคัญทางสถิติ สาเหตุตามความรู้สึกที่เด็กรายงานบ่อยที่สุด คือ ปวดฟัน เสียวฟัน ฟันเป็นรู และมีกลิ่นปาก ส่วนการรับรู้ถึงการเปลี่ยนสีฟันพบว่า กลุ่มซิลเวอร์ไดเอมีนฟลูออไรด์ทั้ง 2 กลุ่ม มีค่าสูงกว่ากลุ่มโซเดียมฟลูออไรด์วาร์นิชอย่างเดียวอย่างมีนัยสำคัญทางสถิติ (p<0.001) อย่างไรก็ตาม การเปลี่ยนสีของฟันไม่ได้เป็นสาเหตุหลักที่ส่งผลกระทบในด้านการยิ้ม หัวเราะ อารมณ์และการเข้าสังคมของเด็ก ดังนั้น การรักษาด้วยฟลูออไรด์ทั้ง 3 วิธี มีผลกระทบต่อคุณภาพชีวิตในมิติสุขภาพช่องปากในกลุ่มเด็กที่มีความเสี่ยงในการเกิดฟันผุสูงไม่แตกต่างกัน และการเปลี่ยนสีของฟันหลังการทาซิลเวอร์ไดเอมีนฟลูออไรด์ไม่ได้เป็นสาเหตุหลักที่มีผลกระทบต่อชีวิตประจำวันของเด็ก
