การฟอกเลือดด้วยวิธีฮีโมไดอะฟิวเตรชัน และผลทางคลินิก

บทคัดย่อ
ปัจจุบันผู้ป่วยไตวายเรื้อรังแนวโน้มจำนวนเพิ่มมากขึ้นทุกปี การรักษาผู้ป่วยในระยะท้ายอาศัยการบัดทดแทนไตด้วยวิธีต่างๆ ได้แก่ การฟอกเลือดด้วยเครื่องไตเทียม การล้างไตทางหน้าท้อง และการปลูกถ่ายไต พบว่าสามารถเพิ่มคุณภาพชีวิตให้กับผู้ป่วยและลดอัตราการเสียชีวิตอย่างชัดเจนโดยเฉพาะอย่างยิ่งการปลูกถ่ายไต แม้ว่าการฟอกเลือดด้วยเครื่องไตเทียมจะมีการพัฒนาเทคนิคต่างๆ มาเป็นเวลานาน ดังนั้นเพื่อเพิ่มประสิทธิภาพในการขจัดของเสียให้เทียบเคียงกับการทำงานของไตมากที่สุด จึงมีการพัฒนาการฟอกเลือดด้วยวิธีฮีโมไดอะฟิวเตรชัน (hemodiafiltration) ซึ่งเป็นวิธีการฟอกเลือดที่เป็นการผสมผสานการขจัดของเสียด้วยกระบวนการแพร่ (diffusion) ร่วมกับการพา (convection) เข้าไว้ด้วยกัน ซึ่งปัจจุบันมีข้อมูลหลักฐานทางคลินิกเกี่ยวกับประโยชน์ต่างๆที่เพิ่มขึ้นอย่างมากมาย อย่างไรก็ตามการฟอกเลือดด้วยวิธีนี้ต้องมีการเตรียมการในหลายอย่างเพื่อให้เกิดประสิทธิผลมากที่สุด เช่น การเตรียมเส้นสำหรับการฟอกเลือด (vascular access) เพื่อให้อัตราการไหลของเลือดเข้าส่เูครื่องไตเทียม (blood flow rate) ให้เพียงพอ, ระบบน้ำแบบบริสุทธิ์มาก (ultrapure dialysis fluid) รวมถึงการทดแทนสารน้ำในปริมาณที่เหมาะสม บทความนี้จะทบทวนหลักการการฟอกเลือดด้วยวิธีฮีโมไดอะฟิวเตรชัน รวมถึงการศึกษาทางคลินิกขนาดใหญ่ ในด้านผลทางคลินิกต่างๆ เช่น อัตราการเสียชีวิต ผลต่อการขจัดของเสียโดยเฉพาะต่อสารโมเลกุลขนาดกลาง ผลต่อระบบหัวใจและหลอดเลือดผลต่อภาวะโภชนาการ เป็นต้น
