บทคัดย่อ

ประเทศไทยได้บรรลุเป้าหมายการกำจัดโรคเรื้อนในระดับที่ไม่เป็นปัญหาสาธารณสุขตามเกณฑ์ขององค์การอนามัยโลกแล้ว แต่บริบทหลังการกำจัดโรคเรื้อนก่อให้เกิดความท้าทายเชิงระบบจากการลดลงของความสำคัญเชิงนโยบายและการจัดสรรทรัพยากร ส่งผลต่อประสิทธิภาพของระบบเฝ้าระวังควบคุมโรค การศึกษานี้มีวัตถุประสงค์เพื่อวิเคราะห์ปัจจัยและแนวโน้มสำคัญที่มีผลต่อระบบเฝ้าระวังควบคุมโรคเรื้อนของประเทศไทย พัฒนาฉากทัศน์อนาคต และสังเคราะห์ระบบเฝ้าระวังควบคุมโรคที่เหมาะสมกับบริบทประเทศไทย โดยใช้ระเบียบวิธี Ethnographic Delphi Futures Research (EDFR) ดำเนินกระบวนการเดลฟาย 3 รอบ โดยผู้เชี่ยวชาญ จำนวน 21 คน วิเคราะห์ฉันทามติด้วยค่า Median และ IQR จัดทำร่างฉากทัศน์ วิพากษ์และยืนยันความเหมาะสมของระบบที่พัฒนาขึ้นโดยผู้เชี่ยวชาญ ผลการศึกษาระบุปัจจัยและแนวโน้มสำคัญ จำนวน 61 ประเด็น ปัจจัยด้านกฎหมาย การเมือง/นโยบาย ได้รับฉันทามติสูง “ความเข้มแข็งของการกำกับเชิงนโยบาย” และ “ระดับการบูรณาการเทคโนโลยีสารสนเทศ” เป็นแกนหลักการพัฒนาฉากทัศน์อนาคต 4 รูปแบบ นำไปสู่การพัฒนารูปแบบระบบเฝ้าระวังควบคุมโรคเรื้อนแบบบูรณาการ ได้แก่ กลไกตรวจจับและตอบสนองอย่างรวดเร็ว การกำกับดูแลเชิงนโยบาย ระบบข้อมูลที่เชื่อมโยง และมาตรการลดการตีตรา ซึ่งสอดคล้องกับบริบทของประเทศไทย จากการวิจัยเสนอให้ยกระดับนโยบายและดำเนินการอย่างต่อเนื่อง ควบคู่กับการใช้เทคโนโลยีบูรณาการข้อมูลร่วมกับเครือข่ายบริการปฐมภูมิ
