บทคัดย่อ

ภาวะหนาวสั่นระหว่างการระงับความรู้สึกเฉพาะส่วนพบได้บ่อยทำให้ผู้ป่วยไม่สุขสบาย รบกวนการวัดสัญญาณชีพ และเพิ่มความเสี่ยงต่อกล้ามเนื้อหัวใจขาดเลือด การศึกษานี้เป็นการทดลองแบบสุ่มมีกลุ่มควบคุม เพื่อหาขนาดยา Dexmedetomidine ที่เหมาะสมในการป้องกันภาวะหนาวสั่นหลังระงับความรู้สึกเฉพาะส่วนในผู้ป่วยผ่าตัดคลอดระหว่างเดือนมิถุนายนถึงเดือนสิงหาคม 2568 และประเมินผลข้างเคียง ผู้ป่วย 140 ราย อายุ 18 - 45 ปี American Society of Anesthesiologists (ASA) I-II ได้รับการระงับความรู้สึกเฉพาะส่วน แบ่งเป็น 3 กลุ่ม ได้แก่ N group (Dexmedetomidine 0.75 ไมโครกรัม/กิโลกรัม, n = 30), L group (Dexmedetomidine 10 ไมโครกรัม, n = 55) และ C group (น้ำเกลือ, n = 55) ระหว่างการผ่าตัดและพักฟื้นติดตามภาวะหนาวสั่น สัญญาณชีพ ระดับความรู้สึกตัว และผลข้างเคียง พบภาวะหนาวสั่นใน N group 1 ราย (ร้อยละ 3.3)
ซึ่งน้อยกว่า L group 12 ราย (ร้อยละ 21.8) อย่างมีนัยสำคัญ (p = 0.024) อย่างไรก็ตามยาทั้งสองกลุ่มมีประสิทธิผลที่ดีในการลดภาวะหนาวสั่นเมื่อเปรียบเทียบกับ C group ที่พบภาวะหนาวสั่น 23 ราย (ร้อยละ 41.8) อย่างมีนัยสำคัญ (p < 0.001 และ p = 0.024 ตามลำดับ) ผลข้างเคียงเรื่องหัวใจเต้นช้าพบใน N group และ L group มากกว่า C group อย่างมีนัยสำคัญโดย N group มากที่สุด ส่วนภาวะความดันโลหิตต่ำเกิดมากในทุกกลุ่ม ผู้วิจัยแนะนำ Dexmedetomidine ขนาด 0.75 ไมโครกรัมต่อกิโลกรัม เนื่องจากป้องกันภาวะหนาวสั่นระหว่างการผ่าตัดคลอดภายใต้การระงับความรู้สึกเฉพาะส่วนได้ดีกว่ายาขนาดต่ำอย่างมีนัยสำคัญ
