การสอบสวนผู้ป่วยโรคซิลิโคสิส 1 ราย สถานประกอบการผลิตภัณฑ์ท่อคอนกรีต จังหวัดปทุมธานี วันที่ 29 - 30 มกราคม 2567

บทคัดย่อ
วันที่ 8 มกราคม 2567 สำนักงานป้องกันควบคุมโรคที่ 4 จังหวัดสระบุรี ร่วมกับสำนักงานสาธารณสุขจังหวัดปทุมธานี และหน่วยงานที่เกี่ยวข้องในพื้นที่ ดำเนินการสอบสวนผู้ป่วยโรคซิลิโคสิส จำนวน 1 ราย เพศชาย วัตถุประสงค์เพื่อยืนยัน การวินิจฉัย อธิบายลักษณะการเกิดโรคจากการสัมผัสซิลิกา ค้นหาสาเหตุปัจจัยที่ก่อให้เกิดโรค และเสนอมาตรการป้องกัน โรคซิลิโคสิส ศึกษาระบาดวิทยาเชิงพรรณนา ทบทวนเวชระเบียนผู้ป่วยยืนยัน ค้นหาผู้ป่วยเพิ่มเติม ศึกษาสภาพแวดล้อม และเก็บตัวอย่างฝุ่นซิลิกา (silica crystalline) เก็บฝุ่นโดยใช้ Personal air sampling pump วิธีการตรวจ NIOSH Methods No.7601 วิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ยและร้อยละโดยปริมาตร (%volume) ผลการสอบสวนผู้ป่วยชายไทย อายุ 52 ปี อาชีพพนักงานเจียรตกแต่งผลิตภัณฑ์ท่อคอนกรีต ระยะเวลาทำงาน 28 ปี พบปอดผิดปกติ ตามเกณฑ์มาตรฐานของ ILO International classification of radiographs of pneumoconioses ระดับ Profusion 2/1 แพทย์วินิจฉัย Interstitial pulmonary disease ,unspecified, Work-related condition และSilicosis การค้นหาผู้ป่วยเพิ่มเติมพบปอดผิดปกติ 3 ราย ส่งให้แพทย์ผู้เชี่ยวชาญอ่านภาพถ่ายรังสีทรวงอกไม่เข้าได้กับโรคซิลิโคสิสทั้งหมด สภาพแวดล้อมสถานที่ทำงานเป็นอาคาร โครงเหล็กหลังคาสูง ปูพื้นด้วยปูนซีเมนต์ พื้นที่ปฏิบัติงาน 3 พื้นที่ ได้แก่ พื้นที่ผสมคอนกรีต อาคารการผลิต และอาคารตรวจสอบคุณภาพ กระบวนการผลิตท่อคอนกรีต 7 ขั้นตอน ผลการประเมินความเสี่ยงทางสุขภาพสูงมากในขั้นตอนตกแต่งแห้ง โดยผู้ปฏิบัติงานใช้มือจับหินทรายขัดให้ท่อคอนกรีตมีผิวเรียบ สัมผัสฝุ่นซิลิกาตลอดระยะเวลาทำงาน ผลการตรวจวัดฝุ่นซิลิกาเกินขีดจำกัดความเข้มข้นของสารเคมีอันตราย แพทย์แนะนำให้ปรับเปลี่ยนตำแหน่งงานให้เหมาะสม ลดการสัมผัสฝุ่นซิลิกา เน้นการสวมใส่มีการสวมอุปกรณ์คุ้มครองความปลอดภัยส่วนบุคคล ที่ถูกต้องตลอดระยะเวลาการทำงาน เฝ้าระวังกลุ่มเสี่ยง ตรวจพิเศษภาพถ่ายรังสีทรวงอกและตรวจสมรรถภาพปอดอย่างน้อยปีละ 1 ครั้ง ติดตามผลการตรวจสุขภาพประจำปีเพื่อรับการวินิจฉัยรักษาที่ถูกต้อง การใช้น้ำดักจับฝุ่นซิลิกาด้วยละอองน้ำบริเวณเครื่องมือที่ใช้ทำงาน หรือติดตั้งเครื่องดูดสุญญากาศติดตั้งไปกับเครื่องเจียรผลิตภัณฑ์
