การประเมินระบบเฝ้าระวังโรคเมลิออยโดสิส ในโรงพยาบาลเขตสุขภาพที่ 4 ปี พ.ศ. 2561 - 2563

บทคัดย่อ

การศึกษานี้เป็นการวิจัยแบบผสมผสานที่รวมการศึกษาเชิงปริมาณและคุณภาพ เพื่อประเมินระบบเฝ้าระวังโรคเมลิออยโดสิส ในเขตสุขภาพที่ 4 โดยสุ่มสถานพยาบาล 6 แห่งจากโรงพยาบาลศูนย์ โรงพยาบาลทั่วไป และโรงพยาบาลชุมชน รวมถึงการทบทวนเวชระเบียนผู้ป่วย 388 รายย้อนหลัง 3 ปี (พ.ศ. 2561 - 2563) พร้อมสัมภาษณ์บุคลากรทางการแพทย์ที่เกี่ยวข้อง 29 ราย ได้แก่ ผู้บริหาร หน่วยรักษาพยาบาล เจ้าหน้าที่ระบาดวิทยา เจ้าหน้าที่เวชสถิติ และเจ้าหน้าที่ห้องปฏิบัติการ
ผลการศึกษาพบว่ามีการรายงานโรคเมลิออยโดสิสต่ำกว่าความเป็นจริง ค่าความไว ร้อยละ 39.0 ค่าพยากรณ์บวก ร้อยละ 76.5 ข้อมูลสามารถเป็นตัวแทนด้านบุคคล และด้านสถานที่ ในพื้นที่ที่มีผู้ป่วยน้อยไม่พบการรายงาน ด้านเวลาป่วยไม่สามารถเป็นตัวแทนได้ ค่าความทันเวลาค่ามัธยฐาน 29 วัน (0 - 592 วัน) ข้อมูลบางตัวแปร เช่น อายุและวันเริ่มป่วยมีความคลาดเคลื่อนจากการสัมภาษณ์เชิงลึกระบบสามารถตอบสนองได้ดี นอกจากนี้พบว่า เจ้าหน้าที่บางส่วนไม่รู้จักโรคเมลิออยโดสิส และระบบรายงานโรค (รง.506) แม้ว่าเจ้าหน้าที่ผู้รับผิดชอบโดยตรงจะคุ้นเคยกับระบบเฝ้าระวังนี้ แต่พบปัญหาความไม่สมบูรณ์ของข้อมูล เช่น ความซ้ำซ้อน ข้อมูลไม่ถูกต้อง และจำนวนเจ้าหน้าที่ไม่เพียงพอ ข้อเสนอแนะ คือควรทบทวนกระบวนการเฝ้าระวังในทุกสถานพยาบาล สร้างเสริมองค์ความรู้โรคเมลิออยโดสิสและนิยามการรายงานโรคในระบบเฝ้าระวังของกองระบาดวิทยาแก่เจ้าหน้าที่สาธารณสุข จัดหาชุดตรวจวินิจฉัยชนิดเร็วเพื่อลดความล่าช้าของการตรวจยืนยัน นำผลทางห้องปฏิบัติการมาใช้ร่วมในการตรวจจับ การแสดงผลข้อมูล (dashboard) เพื่อใช้ประโยชน์สนับสนุนในการเฝ้าระวังควบคุมโรค พร้อมทั้งพัฒนาระบบ hospital information system (HIS) ให้สามารถเพิ่มวันเริ่มป่วยและคัดกรองอาการได้
