ลักษณะทางคลินิก ผลการรักษา และปัจจัยที่เกี่ยวข้องกับข้ออักเสบติดเชื้อแบคทีเรีย ชนิดแกรมบวกและแกรมลบที่โรงพยาบาลหนองคาย

บทคัดย่อ
เป็นการวิจัยเชิงพรรณนาแบบย้อนหลัง โดยเก็บข้อมูลจากการทบทวนเวชระเบียนของผู้ป่วยข้ออักเสบติดเชื้อที่เข้าเกณฑ์ Newman criteria ระดับ A หรือ B ตั้งแต่ปีพ.ศ. 2559-2566 วัตถุประสงค์เพื่อศึกษาลักษณะทางคลินิก ผลการรักษา และปัจจัย
ที่เกี่ยวข้องกับข้ออักเสบติดเชื้อแบคทีเรียชนิดแกรมบวกและแกรมลบที่โรงพยาบาลหนองคาย เปรียบเทียบตัวแปรจัดกลุ่มโดยใช้สถิติ Chi-square test หรือ Fisher’s exact test และตัวแปรต่อเนื่องโดยใช้สถิติ Unpaired t-test หรือ Mann–Whitney U-test แล้วนำเฉพาะปัจจัยที่ p < 0.10 มาวิเคราะห์ต่อโดยใช้สถิติ Multiple logistic regression พบผู้ป่วยที่มีคุณสมบัติเข้าในการวิจัยทั้งหมด 126 ราย เป็นผู้ป่วยข้ออักเสบติดเชื้อแบคทีเรียชนิดแกรมบวก 90 ราย (ร้อยละ 71.4) และแกรมลบ 36 ราย (ร้อยละ 28.6) โดยผู้ป่วยข้ออักเสบติดเชื้อแบคทีเรียชนิดแกรมลบส่วนใหญ่เกิดจากเชื้อ Burkholderia pseudomallei (20 ราย, ร้อยละ 15.9) ประกอบอาชีพรับจ้าง ทำนาหรือเกษตรกรรม (p = 0.025, adjusted OR 3.140) มีโรคตับ (p = 0.090, adjusted OR 2.898) มีอาการไข้ (p = 0.117, adjusted OR 2.633) มีการติดเชื้อในกระแสเลือด (p = 0.199, adjusted OR 2.014) และใส่ท่อช่วยหายใจ (p = 0.160, adjusted OR 2.365) ในขณะที่ผู้ป่วยข้ออักเสบติดเชื้อแบคทีเรียชนิดแกรมบวกมักมีโรคข้อร่วมด้วย (p = 0.025, adjusted OR 0.241) โดยสรุปจากการศึกษานี้แนะนำว่า ผู้ป่วยข้ออักเสบติดเชื้อแบคทีเรียที่ประกอบอาชีพรับจ้าง ทำนาหรือเกษตรกรรมมีโรคตับ สงสัยมีการติดเชื้อในกระแสเลือด ควรพิจารณาให้ยาปฏิชีวนะครอบคลุมเชื้อแบคทีเรียชนิดแกรมลบโดยเฉพาะเชื้อเมลิออยโดสิส ในขณะที่ผู้ป่วยข้ออักเสบติดเชื้อแบคทีเรียที่มีโรคข้อร่วมด้วย ควรพิจารณาให้ยาปฏิชีวนะครอบคลุม
