ผลกระทบต่อผู้ประสบปัญหาจากโรคเรื้อนหลังการบูรณาการนิคมโรคเรื้อนให้เป็นชุมชนทั่วไปในประเทศไทย

บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์คุณภาพชีวิตและการตีตรา ตลอดจนผลกระทบต่อผู้ประสบปัญหาจากโรคเรื้อน ภายหลังการบูรณาการนิคมโรคเรื้อนให้เป็นส่วนหนึ่งของชุมชนทั่วไปในประเทศไทย โดยใช้วิธีวิจัย ทั้งเชิงปริมาณและเชิงคุณภาพ เชิงปริมาณ กลุ่มตัวอย่างประกอบด้วยผู้ประสบปัญหาจากโรคเรื้อนในหมู่บ้านที่เคยเป็นนิคม จำนวน 283 คน และชุมชนรอบนอก จำนวน 270 คน ใช้แบบประเมินคุณภาพชีวิตและแบบวัดการตีตรา วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ร้อยละ ค่าเฉลี่ย และค่าเบี่ยงเบนมาตรฐาน ผลการศึกษา พบว่า กลุ่มผู้ประสบปัญหาจากโรคเรื้อน มีคุณภาพชีวิตอยู่ในระดับดีในทุกด้าน โดยด้านร่างกายมีค่าเฉลี่ยสูงสุดที่ 2.83 รองลงมาคือ ด้านสัมพันธภาพทางสังคมและสิ่งแวดล้อม 2.80 ด้านจิตใจมีค่าเฉลี่ยต่ำสุดที่ 2.75 และคุณภาพชีวิตโดยรวมมีค่าเฉลี่ย 2.77 ด้านการตีตรา พบว่า ร้อยละ 60.4 หากสมาชิกในครอบครัวเป็นโรคเรื้อน จะไม่รู้สึกด้อยหรืออับอาย แต่ยังมีร้อยละ 47.8 ที่เห็นว่าคนในชุมชนอาจปฏิเสธ การเยี่ยมเยียนครอบครัวของผู้ที่เป็นโรคเรื้อน และร้อยละ 47.0 เห็นว่าคนในชุมชนอาจหลีกเลี่ยงการพบปะพูดคุยกับผู้ที่เป็นโรค เชิงคุณภาพ เก็บข้อมูลจากการสัมภาษณ์เชิงลึกและการสนทนากลุ่มกับตัวแทนหน่วยงานที่เกี่ยวข้องในพื้นที่ จำนวน 34 คน และผู้ประสบปัญหาจากโรคเรื้อน จำนวน 54 คน ผลการศึกษา พบว่า ผู้ประสบปัญหาจากโรคเรื้อนได้รับสิทธิ ด้านที่อยู่อาศัยจากกรมธนารักษ์ มีการปรับปรุงที่พักเพื่ออยู่อาศัยและทำมาหากิน การเข้าถึงบริการสุขภาพเป็นไปตามสิทธิที่ได้รับ ด้านสังคมและเศรษฐกิจ พบว่าการขยายตัวของเมืองทำให้เกิดอาชีพหลากหลาย และผู้ประสบปัญหาจากโรคเรื้อนสามารถกลมกลืน เข้ากับชุมชนรอบข้างได้
ที่สำคัญ หน่วยงานภาครัฐมีบทบาทหลักในการบูรณาการและสนับสนุนอย่างต่อเนื่อง โดยการให้บริการและสวัสดิการต่าง ๆ เช่นเดียวกับประชาชนในพื้นที่อย่างเท่าเทียมและเสมอภาค ทำให้เกิดความเข้าใจในการอยู่ร่วมกัน ส่งผลให้ลดการตีตราระหว่างชุมชน ผู้ประสบปัญหาจากโรคเรื้อนและหน่วยงานภาครัฐ นำไปสู่การบรรลุเป้าหมาย Zero Leprosy ตามยุทธศาสตร์ขององค์การอนามัยโลก อย่างไรก็ตาม โรงพยาบาลส่งเสริมสุขภาพประจำตำบลต้องการ การสนับสนุนความรู้ในการดูแลแผลและการป้องกันความพิการในผู้ประสบปัญหาจากโรคเรื้อน ดังนั้น จึงเสนอให้กรมควบคุมโรคกำหนดเป็นจุดเน้นให้กับหน่วยงาน
