การกินยาวาร์ฟารินตามคำสั่งแพทย์อย่างเคร่งครัดของผู้ป่วยโรคหัวใจและหลอดเลือด โรงพยาบาลพระนารายณ์มหาราช จังหวัดลพบุรี

บทคัดย่อ
การศึกษาเชิงพรรณนามีวัตถุประสงค์เพื่อประเมินการกินยาวาร์ฟารินตามคำสั่งแพทย์อย่างเคร่งครัด และหาความสัมพันธ์ระหว่างความรู้ ทัศนคติ แรงสนับสนุนทางสังคมกับการกินยาวาร์ฟารินฯ ของผู้ป่วยโรคหัวใจและหลอดเลือด โรงพยาบาลพระนารายณ์มหาราชจังหวัดลพบุรี ระหว่างเดือนมีนาคม - มิถุนายน 2565 จากการสัมภาษณ์และจากประวัติการรักษาของผู้ป่วย 250 คน โดยใช้สถิติเชิงพรรณนา และการวิเคราะห์โลจิสติกส์แบบทวินาม ที่ระดับนัยสำคัญ 5% ผลการศึกษาพบว่า ร้อยละ 61.2 กินยาวาร์ฟารินตามคำสั่งแพทย์อย่างเคร่งครัด ร้อยละ 74.0, 26.4 และ 88.8 มีความรู้ ทัศนคติ และแรงสนับสนุนทางสังคมในระดับดี ตามลำดับ ร้อยละ 10.8 ดื่มแอลกอฮอล์ ผู้ที่ไม่ดื่มกินยาฯเคร่งครัดดีกว่าผู้ที่ดื่ม (Adjusted Odds Ratio; AOR =3.73, 95%CI:1.35-10.28) ผู้ที่เริ่มกินยา 2-5 ปี กินยาฯอย่างเคร่งครัดดีกว่าผู้ที่กินยามานาน (AOR=3.01, 95%CI:1.53-5.95) ผู้ที่มีความรู้ในระดับดีมากกินยาฯอย่างเคร่งครัดดีกว่าผู้มีความรู้ในระดับน้อย (AOR=4.09, 95%CI:1.24-13.51) ผู้ที่มีทัศนคติที่ดีกินยาอย่างเคร่งครัดมากกว่า(AOR=3.16, 95%CI=1.49-6.69) แต่แรงสนับสนุนทางสังคมไม่มีความสัมพันธ์กับการกินยาฯอย่างเคร่งครัด (AOR=1.54, 95%CI: 0.56-4.19) เพื่อให้ผู้ป่วยกินยาอย่างเคร่งครัด บุคลากรสาธารณสุขต้องเน้นย้ำเรื่องความรู้ ทัศนคติในการกินยาทุกครั้งที่มารับบริการ ต้องมีการวางแผนร่วมกันกับญาติผู้ใกล้ชิดเพื่อให้ผู้ป่วยลด ละ เลิก พฤติกรรมสุขภาพที่ไม่พึงประสงค์
