การประเมินความเหมาะสมของขนาดการสั่งใช้ยา Metformin ในผู้ป่วยโรคเบาหวาน ชนิดที่ 2 โรงพยาบาลอินทร์บุรี

บทคัดย่อ
การศึกษาเชิงพรรณนาแบบย้อนหลัง เพื่อประเมินความเหมาะสมของขนาดการสั่งใช้ยา metformin ในผู้ป่วยโรคเบาหวาน ชนิดที่ 2 และศึกษาอุบัติการณ์ metformin-associated lactic acidosis (MALA) ในผู้ป่วยโรคเบาหวาน ชนิดที่ 2 โรงพยาบาลอินทร์บุรี โดยวิเคราะห์ข้อมูลผู้ป่วยที่ได้รับการวินิจฉัยด้วยโรคเบาหวาน (รหัสโรค ICD10 E11.0-E11.9) และรับการรักษาแผนกผู้ป่วยนอก โรงพยาบาลอินทร์บุรี ระหว่างวันที่ 1 ตุลาคม 2564 ถึง 30 กันยายน 2565 พบว่าผู้ป่วยโรคเบาหวานจำนวน 2,596 คน จากการติดตามรวม 10,580 ครั้ง พบขนาดการสั่งใช้ยา metformin อย่างสมเหตุผลร้อยละ 96.12 (n=10,169) พบผู้ป่วยเกิดภาวะไตวายเฉียบพลัน ร้อยละ 0.48 ครั้งของการติดตาม (n=51) นอกจากนี้ยังพบผู้ป่วยเกิดอาการไม่พึงประสงค์รุนแรงจากยา metformin คือ ภาวะ metformin-associated lactic acidosis (MALA) คิดเป็นอุบัติการณ์ร้อยละ 0.03 ครั้งของการติดตาม (n=3) (115.56 ราย ต่อ 100,000 ราย-ปี ที่ได้รับยา metformin) โดยผู้ป่วยทั้ง 3 รายเกิดภาวะไตวายเฉียบพลันและมีผู้ป่วย 1 รายได้รับการบำบัดทนแทนไต จากข้อมูลการศึกษานี้ บุคลากรทางการแพทย์ควรตระหนักเรื่องการใช้ยาอย่างสมเหตุผล รวมทั้งควรสร้างแนวทางปฏิบัติเพื่อส่งเสริมการใช้ยาอย่างสมเหตุผล เพื่อให้ผู้ป่วยปลอดภัยจากการใช้ยา
