ปัจจัยที่ส่งผลต่อระยะเวลาการเข้าสู่กระบวนการรักษาล่าช้าของผู้ป่วยโรคหลอดเลือดสมอง โรงพยาบาลสีคิ้ว จังหวัดนครราชสีมา ในปีงบประมาณ 2563-2565

บทคัดย่อ
การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่ส่งผลต่อการเข้าสู่กระบวนการรักษาล่าช้าของผู้ป่วยโรคหลอดเลือดสมองในอำเภอสีคิ้ว จังหวัดนครราชสีมา ใช้วิธีการศึกษาตามรุ่นย้อนหลัง กลุ่มตัวอย่าง ได้แก่ ผู้ป่วยอายุมากกว่า 15 ปี ที่ได้รับการวินิจฉัยโรคหลอดเลือดสมอง (ICD-10 I60-I69) และเข้ารับการรักษาในโรงพยาบาลสีคิ้ว จังหวัดนครราชสีมา
ในปี พ.ศ. 2563 - 2565 จำนวน 291 ราย สถิติที่ใช้ในการวิเคราะห์ข้อมูล ได้แก่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ การวิเคราะห์ Multivariable logistic regression ผลการวิจัยพบว่า 1) ปัจจัยจากตัวผู้ป่วยและสิ่งแวดล้อม ด้านที่อยู่ (F = 4.358,
p-value = 0.013) การเป็นโรค DLP (F = 5.748, p-value = 0.017) ด้านอาการผู้ป่วย (F = 55.382, p-value = 0.003) ด้านการเป็นโรค DM (F = 5.748, p-value = 0.017) ส่งผลต่อการเข้าสู่กระบวนการรักษาล่าช้าของผู้ป่วยโรคหลอดเลือดสมองในอำเภอสีคิ้ว จังหวัดนครราชสีมา 2) ปัจจัยจากระบบบริการการแพทย์ฉุกเฉิน ด้านวิธีการมาโรงพยาบาล (F = 37.563, p-value < 0.001) และช่วงเวลาการเข้ารับการรักษา (F = 8.903, p-value = 0.003) ส่งผลต่อการเข้าสู่กระบวนการรักษาล่าช้าของผู้ป่วยโรคหลอดเลือดสมองในอำเภอสีคิ้ว จังหวัดนครราชสีมา 3) ระบบการให้บริการของโรงพยาบาลด้าน Door to Triage Time (F = 34.969, p-value < 0.001) Triage to Physician contact time (F = 16.047, p-value < 0.001) และด้าน Activate Stroke care (F = 9.845, p-value = 0.002) ส่งผลต่อการเข้าสู่กระบวนการรักษาล่าช้าของผู้ป่วยโรคหลอดเลือดสมองในอำเภอสีคิ้ว จังหวัดนครราชสีมา
