Nursing Care of Tonsillectomy in Obese Children with Obstructive Sleep Apnea: Case Study

Abstract
Tonsillectomy in pediatric patient who was indicated of obstructive sleep apnea syndrome and obesity is at an in-creased risk for respiratory system complications when compared with the surgery under indication of frequent in-fection. Serious and life-threatening complications can occur both during and after surgery. Therefore, proper and comprehensive nursing care helps to reduce the incidence of complications that may occur. The objective of this study was to determine the effectiveness of nursing care of tonsillectomy in child who is obese and obstructive sleep apnea. An appropriate care can be standardized across all inpatient units and use for further care plan. A case study was a 7 year old child. He was presented at the hospital with harsh sound breathing, snoring and had frequently been startled in the middle of night. On history taking and physical examination found body mass index of 38.89, adenotonsillar hypertrophy and obstructive sleep apnea. The bilateral tonsillectomy under anesthetic pro-cedure was then scheduled. The nursing care for physical and psychological preparations of pediatric patient and family in the preoperative phase and the management on breathing, bleeding, and pain relief within the first 24 h after surgery are important to reduce the complication on both peri- and post-operation. Patient could breathe easi-ly, no choking, slight bleeding from the surgical wound, slight sore throat and did not have any serious complica-tions. In this study, the comprehensive nursing on both pre- and post-operation together with the well education for home care helps to increase the efficacy of treatment, patient recovered well, did not has respiratory complication, surgical wound infection or secondary bleeding, moreover, it also promotes the quality of life for patient and family.
