Identification of Multiple Antibodies in a Thai Patient

Abstract

บทคัดย่อ ผู้ป่วยหญิงไทยคู่ อายุ 76 ปี เข้ารับการรักษาด้วยภาวะซีด แพทย์สั่งจอง red cells 2 ยูนิต ผลการตรวจทางห้องปฏิบัติการพบว่า ผู้ป่วยมีหมู่เลือด O, Rh(D) positive ผลการตรวจกรองแอนติบอดีในพลาสมาโดยวิธี column agglutination test (CAT) ให้ผลบวก (3+) กับ screen cells ทั้ง 3 cells จึงทำการตรวจหาชนิดของแอนติบอดีโดยวิธี conventional tube test (CTT) อ่านผลที่อุณหภูมิห้องพบว่าเป็น anti-M เมื่ออ่านผลหลังจาก incubate ที่ 37 ํC นาน 30 นาที พบว่าเป็น anti-Mia และผลการตรวจ indirect antiglobulin test (IAT) พบว่า ให้ผลบวกกับ panel cells ทุกราย ยกเว้น autocontrol แสดงว่าผู้ป่วยน่าจะมี multiple antibodies จึงทำการตรวจเพิ่มเติมโดยการทำantigen typing และเลือก extra cells มาทดสอบ พบว่าผู้ป่วยมี anti-E และ anti-Jka ร่วมด้วย สรุป ผู้ป่วยรายนี้มี anti-M, -Mia, -E และ anti-Jka ในการหาเลือด M-, Mi(a-), E-, Jk(a-) จำนวน 2 ยูนิต ต้องสุ่มตรวจจากเลือดจำนวน 137 ยูนิต จึงได้เลือดที่ไม่มีแอนติเจนดังกล่าวให้แก่ผู้ป่วย

Abstract: A 76-year-old Thai woman with anemia was admitted to the hospital. Two units of red cells were requested. Laboratory testing results showed that she was group O, Rh(D) positive. Antibody screening test in her plasma by column agglutination test (CAT) was positive (3+) with all 3 screen cells. Antibody identification by conventional tube test (CTT) demonstrated anti-M at room temperature phase. Thereafter, 37 ํC incubation for 30 minutes, anti-Mia was found. Regarding the results of indirect antiglobulin test (IAT), all panel cells except autocontrol showed positive reactions. Hence, multiple antibodies were suspected. To resolve these multiple antibodies, antigen typing and testing with selected panel cells were included. Finally, it was concluded that this patient had anti-M, -Mia, -E and anti-Jka. In order to find two compatible red cell units lacking of those antigens, at least 137 random units are required.
