ภาวะแทรกซ้อนทางระบบประสาทในมะเร็งระบบโลหิตของผู้ใหญ่ ในโรงพยาบาลสงขลานครินทร์ : ความชุกและลักษณะทางคลินิก

บทค้ดย่อ : วัตถุประสงค์ : เพื่อศึกษาความซุกและลักษณะคลินิกของภาวะแทรกซ้อนทางระบบประสาท และหาความสัมพันธ์ระหว่างชนิดของกาวะแทรกซ้อนที่เกิดขึ้นกับชนิดของมะเร็งระบบโลหิตแต่ละอย่าง แบบการศึกษา : การศึกษาเชิงพรรณนาแบบย้อนหลัง สถานที่ศึกษา : โรงพยาบาลสงขลานครินทร์ วัสดุและวิธีการ : ผู้ป่วยอายตั้งแต่ 15 ปีขึ้นไปได้รับการวิจัยฉัยว่าเป็นเม็ดเลือดขาว มะเร็งต่อมนำเหลือง และ mutiple myeloma รับไว้ไว้ในโรงพยาบาลระหว่างเดือนมกราคม พ.ศ. 2530 และ ธันวาคม พ.ศ. 2539 การวินิจฉัยกาวะแทรกซ้อนทางระบบประสาทอาศัยลักษณะทางคลินิกร่วมกับการตรวจทางห้องปฏิบัติการ
เก็บข้อมูลจากเวชระเบียนของผู้ป่วยเกี่ยวกับอายุ เพศ ชนิดของมะเร็ง ระยะของโรค อาการและอาการแสดงของกาวะแทรกซ้อนทางระบประสาท การตรวจวินิจฉัยเพิ่มเติม การรักษาและผลการรักษา ผลการศึกษา : จากผู้ป่วยมะเร็งระบบโลหิต ทั้งหมด 1,693 คน ค้นเวชระเบียนได้ 1,545 คน (91.3%) พบที่มีกาวะแทรกซ้อนทางระบบประสาท 173 คน (11.2%) จำแนกได้ดังนี้ ภาวะเลือดออกในกะโหลกศีรษะ 39.3% พยาธิสภาพที่เยื่อหุ้มสมอง 23.7% ภาวะไขสันหลังถูกกดทับ 16.8% ภาวะก้อนในสมอง 6.9% ภาวะเส้นประสาทส่วนปลายถูกกดทับ 4.6% เมื่อจำแนกตามชนิดของมะเร็ง ความทุกของกาวะแทรกซ้อนทางระบบประสาทในผู้ป่วยมะเร็งเม็ดเลือดขาวเฉียบพลันชนิด lymphoblast (ALL) 13:0% มะเร็งเร็ดเลือดขาวเฉียบพลันชนิด non-lymphoblast (ANLL) 116% มะเร็งเร็งเลือดขาวเรือรังชนิด myeloid (CML) 16.1% มะเร็งต่อมน้ำเหลืองชนิด non-Hodgkin (NIHL) 10.3% มะเร็งต่อมน้ำเหลืองชนิด Hodgkin (HD) 6.9% และ multiple myeloma (MMM) 9.9% min) laisia) รวมภาวะเลือดออกในกะโหลกศีรษะ ความซุกของภาวะแทรกช้อนทางระบบประสาทในมะเร็งระบโลหิตชนิดต่างๆ เท่ากับ ร้อยละ 87, 19,40, 97, 56 และ 92 ตามลำดับ ชนิดของมะเร็งระบบโลหิตมีความสัมพันธ์กับชนิดของภาวะแทรกซ้อนทางระบบประสาทอย่างมีนัยสำคัญทางสถิติ (p < 0.001) โดยภาวะเลือดออกในกะโหลกศีรษะ พบใน ANLL 9.6% พยาธิสภาพ ที่เยื่อหุ้มสมองพบใน ALL 7.5% และใน NHL 3.7% ภาวะไขสันหลังถูกกดทับพบใน MM 6.66% HD 2.8% และ NHL 2.3% สรุป : ภาวะแทรกซ้อนทางระบบประสาทในผู้ป่วยมะเร็งระบบโลหิตเป็นภาวะที่มีความสำคัญ และสัมพันธ์กับชนิดของมะเร็งการนิจฉัยได้รวดเร็วตั้งแต่แรกเริ่ม จะทำให้ตนกรถให้การรักษาที่ถูกต้องและลดความพิการได้
