Hemolytic Disease of the Newborn due to Anti-c and Anti-E

Abstract

Abstract: A case of hemolytic disease of the newbom (HDN) due to anti-c and anti-E is reported. A group O, R R female infant was born to a group O, RR woman. Anti-c and anti-E were detected in the maternal serum. Adsorption the matemal serum with R r cell and rr cell reved no anti-cE (Rh27) in the maternal serum. Her mother processed preformed anti-c and anti-E by previous pregnancy and /or 10 units of platelet concentrate transfusion. Hyperbilinubinemia was noted at the first day of life. The direct antiglobulin test was 3'. Anti-c and anti-E were demonstrated in the eluate from the baby's red cells. At the age of 3 days, the baby was severe anemic and marked jaundice (hematocrit 28%, total bilirubin 24 42 mg/dl). Despite phototherapy, the total bilirubin level increased. An exchange transfusion with group O, R packed red cells suspended in group O, fresh frozen plasma/ had been performed which resulted in a prompt decrease in total bilirubin. She was improved at the age of 4 days (hematocrit 54%, total bilubin 11.1 mg/al, and was discharged at 5 days of age without complication. Follow-up the baby at the age of 4 months showed that she was alive and in good health. No neurological complication was detected.
