ศึกษาหาเกณฑ์มาตรฐานของปริมาตร Packed Red Cells และ Fresh Frozen Plasma

โลหิต จาก ผู้ บริจาค ภาย หลัง เจาะ เก็บ ใน ถุง บรรจุ โลหิต จะ ถูก ปั่น แยก เป็น ส่วน ประกอบ โลหิต ชนิด ต่างๆ เพื่อ
นำ ไป ใช้ รักษา ผู้ป่วย โดย ถุง บรรจุ โลหิต ที่ ศูนย์ บริการ โลหิต แห่ง ชาติ (NBC) ใช้ มี 2 ขนาด คือ ขนาด เจาะ เก็บ โลหิต 450
mL และ 350 mL ทำให้ ส่วน ประกอบ โลหิต ชนิด Packed Red Cells (PRC) และ Fresh Frozen Plasma (FFP) ภาย
หลัง การ ปั่น แยก จาก ถุง 2 ขนาด มี ปริมาตร ที่ แตก ต่างกัน โดย ปริมาตร มาตรฐาน สากล มี เฉพาะ ถุง ขนาด 450 mL และ
ประเทศ ไทย ไม่ มี เกณฑ์ ปริมาตร กำหนด ไว้ เป็น มาตรฐาน ศูนย์ บริการ โลหิต แห่ง ชาติ จึง ดำเนิน การ ศึกษา หา ค่า ปริมาตร
ของ ส่วน ประกอบ โลหิต PRC และ FFP แล้ว นำ มา คำนวณ ทาง สถิติ และ เปรียบ เทียบ ค่า ที่ ได้ จาก การ ศึกษา นี้ กับ เกณฑ์
มาตรฐาน สากล จาก ผล การ ศึกษา สามารถ กำหนด เกณฑ์ ปริมาตร PRC จาก โลหิต เจาะ เก็บ ใน ถุง ขนาด 450 mL และ
350 mL เท่ากับ 200 - 330 mL และ 160 - 260 mL ตาม ลำดับ และ เกณฑ์ ปริมาตร FFP กำหนด ไว้ ขนาด เดียว คือ มาก
กว่า หรือ เท่ากับ 150 mL สำหรับ โลหิต เจาะ เก็บ ทั้ง 2 ขนาด เกณฑ์ ปริมาตร ที่ ได้ จาก การ ศึกษา นี้ ใช้ เป็น มาตรฐาน ของ ศูนย์
บริการ โลหิต แห่ง ชาติ ใน การ ตรวจ สอบ ปริมาตร ของ PRC และ FFP ที่ เตรียม ได้ รวม ถึง การ ตรวจ รับ เข้า ส่วน ประกอบ
โลหิต จาก สาขา บริการ โลหิต อื่น

Abstract:

After blood collection, blood is separated into blood components for specific treatment. Since
two different sizes of blood bags,350 mL and 450 mL are used at National Blood Centre (NBC), volume
of  Packed Red Cells (PRC) and Fresh Frozen Plasma (FFP) after blood components separation are different.
According to International Guidelines, volume for PRC and FFP prepared from only 450 mL
blood  bag are specified. In Thailand, there was no standard specification on volume for both components.
The NBC studied volume ranges of both components and will be put into statistical value
comparing  with the international reference. The result of standard specification on volume for PRC
in 450 mL and 350 mL blood bag is equivalent to 200- 330 mL and 160- 260 mL respectively. For FFP,
only  one specification for both size of blood bag was ≥  150 mL. The standard specification on
volume will  be used as the reference for the quality control of blood components volume at NBC as well
as for inspection  of incoming blood components.
