การเกิดร่วมกันของมะเร็ง Teratoma และ มะเร็งต่อมน้ำเหลือง ชนิด Diffuse Large B-Cell

การเกิดร่วมกันของมะเร็ง germ cell และมะเร็งต่อมน้ำเหลือง เป็นภาวะที่พบได้น้อยมาก รายงานผู้ป่วยชายอายุ 20 ปี
มาด้วยอาการ ไข้ เลือดกำเดาไหล 3 วันก่อนมาโรงพยาบาล ตรวจร่างกาย พบตับม้ามโต ฟิล์มเอกซเรย์ปอดและสแกนคอมพิวเตอร์ พบว่ามีก้อนที่ช่องอกขนาด 6 x 7.5 x 6 เซนติเมตร ผลชิ้นเนื้อที่ก้อนช่องอกเป็น immature teratoma และผลตรวจเลือดพบว่ามี เกล็ดเลือดต่ำ 11 x 109/L และพบเม็ดเลือดแดงตัวอ่อน (nucleated red blood cell) การตรวจชิ้นเนื้อไขกระดูกพบว่าเป็นมะเร็ง ต่อมน้ำเหลืองชนิด CD20 negative diffuse large B-cell ได้ให้การรักษาด้วยยาเคมีบำบัดสูตร BEC ซึ่งประกอบด้วย bleomycin, etoposide และ cisplatin หลังจากได้ยาเคมีบำบัด 3 รอบ ก้อนที่ช่องอกมีขนาดใหญ่ขึ้นจนกดเส้นเลือดดำใหญ่บริเวณช่องอก และ มีปอดอักเสบติดเชื้อร่วมด้วย ต่อมาเสียชีวิตเนื่องจากปอดอักเสบติดเชื้อ

Abstract:
Lymphoma occurred simultaneously with germ cell tumors is uncommon. We report a 20-year-old Thai male presented with acute fever and one episode of epistaxis for 3 days. The physical examination revealed hepatosplenomegaly. Chest X-ray and CT scan showed anterior mediatinal mass size 6 x 7.5 x 6 cm. The biopsy of mass proved to be immature teratoma. Laboratory testing revealed thrombocytopenia (platelet 11 x 109/L), and nucleated red blood cells in peripheral blood smear. The report of bone marrow biopsy was CD20
negative diffuse large B-cell lymphoma. He received chemotherapy BEC regimen consisting of bleomycin, etoposide, and cisplatin. After 3 cycles of chemotherapy, the mediastinal mass progressed and caused superior vena cava (SVC) obstruction. He was admitted in ICU and died of pneumonia.
