บทคัดย่อ
วาร์ฟารินเป็นยาต้นแบบของยาต้านการแข็งตัวของเลือดชนิดรับประทานในกลุ่มคูมารินที่นิยมใช้กันอย่างแพร่หลายในประเทศไทยเพื่อรักษาและป้องกันการเกิดภาวะอุดตันของหลอดเลือด ในการดูแลผู้ป่วยที่มีข้อบ่งชี้ในการใช้ยาต้านการแข็งตัวของเลือด แต่ปัญหาคือยาวาร์ฟารินเป็นยาที่มีช่วงห่างของขนาดยาที่ให้ผลในการรักษาและขนาดยาที่ทำให้เกิดพิษแคบ และเป็นยาที่มีความเสี่ยงสามารถทำให้เกิดภาวะเลือดออกได้จนถึงขั้นเสียชีวิต ดังนั้นการศึกษานี้จึงมุ่งเน้นพัฒนาระบบผู้เชี่ยวชาญต้นแบบเพื่อช่วยในการให้คำแนะนำแนวทางการปรับขนาดยาวาร์ฟารินในผู้ป่วยสำหรับเภสัชกร โดยขั้นตอนของการพัฒนาระบบเริ่มต้นจากการกำหนดความรู้ด้วยการนำข้อมูลและความรู้ที่รวบรวมได้จากเอกสารหลักฐานทางวิชาการ คู่มือปฏิบัติ และการสัมภาษณ์ผู้เชี่ยวชาญ มาทำการจัดเรียบเรียงและแทนความรู้ให้อยู่ในรูปแบบฐานกฏในรูปของ IF…THEN… จากนั้นใช้ภาษา CLIPS ในการพัฒนาโปรแกรมด้วยวิธีหาเหตุผลแบบไปข้างหน้า และวิธีค้นหากฏแบบลงทางลึกก่อน ผลการทดสอบระบบผู้เชี่ยวชาญต้นแบบพบว่า ระบบสามารถให้คำแนะนำที่ถูกต้อง และเภสัชกรมีความพอใจในการใช้งานระบบ เนื่องจากระบบสามารถให้คำแนะนำที่รวดเร็วดีมากและแม่ยำดี สอดคล้องกับวัตถุประสงค์
