การพลัดตกหกล้มของผู้สูงอายุในสถานบริบาลเอกชน พื้นที่ภาคตะวันออกเฉียงเหนือของประเทศไทย

ทั่วโลกพบการพลัดตกหกล้มเป็นสาเหตุการเสียชีวิตอันดับสองในการบาดเจ็บโดยไม่เจตนา และผู้สูงอายุเป็นกลุ่มที่มีอัตราการเสียชีวิตสูงสุด อย่างไรก็ตาม ไม่ปรากฏมีงานวิจัยการพลัดตกหกล้มของผู้สูงอายุในสถานบริบาลเอกชน โดยเฉพาะอย่างยิ่งในเขตภาคตะวันออกเฉียงเหนือของประเทศไทยซี่งมีสัดส่วนประชากรผู้สูงอายุมากที่สุดในประเทศ การวิจัยแบบผสมผสานนี้ มีวัตถุประสงค์เพื่อหาความชุก ลักษณะการพลัดตกหกล้ม และ ศึกษาปัจจัยที่มีความสัมพันธ์กับการพลัดตกหกล้มของผู้สูงอายุที่อยู่ในสถานบริบาลเอกชน โดยคัดเลือกผู้สูงอายุจำนวน 56 คน จากสถานบริบาล 20 แห่ง ตามเกณฑ์ที่กำหนด เพื่อประเมินข้อมูลเชิงปริมาณเกี่ยวกับการพลัดตกหกล้มในช่วง 6 เดือนที่ผ่านมา และเก็บรวบรวมข้อมูลเชิงคุณภาพด้วยการสัมภาษณ์เชิงลึก โดยผู้ให้ข้อมูลสำคัญคือ ผู้แทนหรือผู้ดูแลในสถานบริบาล ทั้ง 20 แห่ง วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนา และใช้ Firth's penalized logistic regression เพื่อวิเคราะห์ปัจจัยที่มีความสัมพันธ์กับการพลัดตกหกล้มของผู้สูงอายุ วิเคราะห์ข้อมูลเชิงคุณภาพโดยวิธีวิเคราะห์เชิงธีม ผลการศึกษาเชิงปริมาณ พบ ผู้สูงอายุ 5 คนจาก 56 คน (ร้อยละ 8.9, 95% CI: 3.7-20.2) มีการพลัดตกหกล้มในรอบ 6 เดือนที่ผ่านมา โดยเหตุการณ์ส่วนใหญ่เกิดขึ้นที่ห้องนอน และเกิดขึ้นขณะเดินหรือเปลี่ยนท่าทาง ปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับการพลัดตกหกล้มพบปัจจัยเดียว คือ การใช้ยากล่อมประสาท นอกจากนี้ ปัจจัยสิ่งแวดล้อมที่เสี่ยงต่อการพลัดตกหกล้มซึ่งพบมากที่สุด คือ การไม่มีฉากกั้นในห้องน้ำและแสงสว่างไม่เพียงพอ ผลการศึกษาเชิงคุณภาพ พบ ผู้สูงอายุได้รับการดูแลอย่างใกล้ชิดตลอด 24 ชั่วโมง โดยปัจจัยที่เกี่ยวข้องกับการพลัดตกหกล้มประกอบด้วย ผู้ดูแล สิ่งแวดล้อม และ ตัวผู้สูงอายุ โดยสรุป ความชุกของการพลัดตกหกล้มของผู้สูงอายุที่อาศัยอยู่ในสถานบริบาลเอกชน เขตภาคตะวันออกเฉียงเหนือของประเทศไทย อยู่ในระดับต่ำ อย่างไรก็ตาม สถานบริบาลบางแห่งมีสภาพแวดล้อมที่ส่งผลทำให้เกิดการพลัดตกหกล้มของผู้สูงอายุได้
