ความชุกของผลตรวจทางห้องปฏิบัติการของผู้สัมผัสอาหาร และความสัมพันธ์ต่อปัจจัยด้านบุคคลและสิ่งแวดล้อม ในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย

บทคัดย่อ

โรคติดเชื้อทางอาหารเป็นปัญหาสาธารณสุขที่สำคัญระดับโลก โดยองค์การอนามัยโลกประมาณการว่ามีผู้ป่วยปีละ 600 ล้านคน และเสียชีวิต 420,000 ราย ผู้สัมผัสอาหารมีบทบาทสำคัญในการป้องกันโรคดังกล่าว การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความชุกของผลตรวจทางห้องปฏิบัติการที่ผิดปกติในผู้สัมผัสอาหาร และความสัมพันธ์ระหว่างปัจจัยส่วนบุคคลและปัจจัยด้านสิ่งแวดล้อมกับผลตรวจทางห้องปฏิบัติการฯ ดังกล่าว ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย การศึกษาเชิงพรรณนาแบบภาคตัดขวาง โดยการรวบรวมข้อมูลย้อนหลังในช่วงปี พ.ศ. 2563-2567 ของผู้สัมผัสอาหารจำนวน 247 คน ประกอบด้วย เพศ อายุ สัญชาติ อายุการทำงาน ประเภทร้านอาหาร ผลการเพาะเชื้ออุจจาระ ผลภาพถ่ายรังสีทรวงอก และผลตรวจภูมิคุ้มกันไวรัสตับอักเสบเอ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการวิเคราะห์การถดถอยโลจิสติก โดยกำหนดระดับนัยสำคัญทางสถิติที่ 0.05 ผลการศึกษาพบว่า ผู้สัมผัสอาหารส่วนใหญ่เป็นเพศหญิง (ร้อยละ 75.7) มีอายุระหว่าง 25-34 ปี (ร้อยละ 33.2) มีสัญชาติไทย (ร้อยละ 77.3) และมีอายุการทำงานน้อยกว่า 3 ปี (ร้อยละ 76.5) ประเภทสถานประกอบการที่พบมากที่สุด คือ ร้านอาหารแห้ง/บรรจุภัณฑ์ (ร้อยละ 41.3) และร้านค้าปลีก/ร้านสะดวกซื้อ (ร้อยละ 51.0) ความชุกของการตรวจพบเชื้อในอุจจาระ (รวมเชื้อแบคทีเรียและปรสิต) อยู่ที่ที่ร้อยละ 30.3 จากการวิเคราะห์ความสัมพันธ์ พบว่าปัจจัยส่วนบุคคลและปัจจัยด้านสิ่งแวดล้อมที่ศึกษาไม่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับผลตรวจทางห้องปฏิบัติการที่ผิดปกติเพื่อค้นหาปัจจัยเสี่ยงที่แท้จริง สำหรับการบริหาร ควรมีการประเมินความเสี่ยงอย่างรอบด้าน ทั้งนี้ไม่ควรใช้ผลการตรวจที่ผิดปกติเป็นเกณฑ์กีดกันผู้สัมผัสอาหารอย่างถาวร แต่ควรเป็นข้อมูลเพื่อการดูแลรักษาและติดตามผล
