ความสัมพันธ์ระหว่างความรู้สึกเป็นภาระในการดูแลและคุณภาพการนอนหลับของผู้ดูแลผู้สูงอายุติดเตียงในอำเภอสันป่าตอง จังหวัดเชียงใหม่

บทคัดย่อ

วัตถุประสงค์: ศึกษาความสัมพันธ์ระหว่างความรู้สึกเป็นภาระในการดูแลและคุณภาพการนอนหลับของผู้ดูแลผู้สูงอายุติดเตียงในอำเภอสันป่าตอง จังหวัดเชียงใหม่

วิธีการศึกษา: ศึกษาแบบตัดขวางโดยใช้แบบสอบถามผู้ดูแลผู้สูงอายุติดเตียงในอำเภอสันป่าตอง จังหวัดเชียงใหม่ เก็บข้อมูลระหว่างเดือนกุมภาพันธ์ถึงเดือนกรกฎาคม พ.ศ. 2563 ประเมินความรู้สึกเป็นภาระของผู้ดูแลใช้แบบสอบถาม Zarit Burden Interview (ZBI) ประเมินคุณภาพการนอนหลับของผู้ดูแลโดยใช้แบบสอบถาม Pittsburg Sleep Quality Index (PSQI) วิเคราะห์ความสัมพันธ์โดยใช้ Multivariable logistic analysis

ผลการศึกษา: ผู้ดูแลและผู้สูงอายุติดเตียงทั้งหมดจำนวน 136 คู่ ผู้ดูแลมีความรู้สึกเป็นภาระในการดูแลร้อยละ 80.15 จากผู้ดูแลทั้งหมด ค่าเฉลี่ยคะแนน ZBI score 34.65 ± 15.51 คะแนน มีความชุกของคุณภาพการนอนหลับที่ไม่ดีร้อยละ 93.28 ค่าเฉลี่ยคะแนน PSQI score 8.02 ± 1.84คะแนน จากการวิเคราะห์ปัจจัยที่สัมพันธ์กับความรู้สึกเป็นภาระของผู้ดูแลด้วย Multivariable logistic analysis พบว่าปัจจัยเสี่ยงที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติ คือ คุณภาพการนอนหลับที่ไม่ดี สัมพันธ์กับความรู้สึกเป็นภาระมากขึ้น 8.87 เท่า (95%CI = 1.32 - 59.52, p=0.02) และอีกปัจจัยที่เข้าใกล้การมีนัยสำคัญทางสถิติ คือ การที่ผู้ป่วยที่ติดเตียงด้วยสาเหตุจากโรคทางระบบประสาทและสมอง สัมพันธ์กับความรู้สึกเป็นภาระมากขึ้น 5.21 เท่า (95%CI = 0.94 - 28.78, p=0.05)

สรุป: ความรู้สึกเป็นภาระในการดูแลผู้สูงอายุติดเตียงพบได้บ่อยและสัมพันธ์กับคุณภาพการนอนหลับที่ไม่ดีของผู้ดูแล โดยเฉพาะกลุ่มผู้สูงอายุที่ติดเตียงจากโรคทางระบบประสาทและสมอง ดังนั้น ควรให้ความสำคัญในการคัดกรองความรู้สึกเป็นภาระและคุณภาพการนอนหลับของผู้ดูแลในการดูแลผู้สูงอายุกลุ่มนี้
